基于胃气升降理论探讨整肠散穴位贴敷预防胃癌术后功能性排空障碍的效果

摘　　要：目的 探讨基于胃气升降理论的整肠散穴位贴敷预防胃癌术后功能性排空障碍(FDGE)的效果。方法 通过查阅文献,对胃气升降理论进行阐述,并观察胃气升降理论在临床的应用效果。选择2020年1月至2022年4月福建中医药大学附属人民医院胃肠外科胃癌行胃切除术后的住院患者88例,按照随机数表法将患者随机分为对照组和观察组,每组各44例。对照组给予术后常规护理和治疗,观察组在常规护理和治疗的基础上给予整肠散穴位贴敷,干预时间均为15 d,记录胃管的留置时间,评价患者的血钾、胃肠道症状、并发症及不良反应发生情况。结果 干预后观察组胃管留置时间短于对照组(P<0.05),血钾高于对照组,且在正常范围内波动(P<0.05),胃肠道症状评分低于对照组(P<0.05),两组患者均未发生并发症及不良事件。结论 整肠散穴位敷贴有效缩短胃管留置时间,维持血钾浓度在正常水平,改善胃癌术后患者胃肠道症状,疗效显著。本研究基于胃气升降理论探讨了整肠散穴位贴敷的应用效果,为其作用机制提供循证医学证据。Objective To explore the effect of acupoint application of Zhengchang Powder on preventing functional delayed gastric emptying(FDGE)after gastric cancer surgery based on the theory of ascending and descending of stomach qi.Methods The theory of ascending and descending of stomach qi was elaborated by consulting ancient literature,and its clinical application effect was observed.A total of 88 hospitalized patients with gastric cancer after gastrectomy in the Department of Gastrointestinal Surgery of the People’s Hospital Affiliated to Fujian University of Traditional Chinese Medicine from January 2020 to April 2022 were selected and divided into a control group and an observation group according to the random number table,with 44 patients in each group.The control group received routine postoperative care and treatment,while the observation group received the acupoint application of Zhengchang Powder on the basis of routine care and treatment.The intervention duration was 15 days,and the retention time of the gastric tube was recorded.The patient’s blood potassium,gastrointestinal symptoms,complications,and incidence of adverse reactions were evaluated.Results After the intervention,the gastric tube retention time in the observation group was shorter than that in the control group(P<0.05),and the blood potassium was higher than that in the control group,fluctuating within the normal range(P<0.05).The gastrointestinal symptom score was lower than that in the control group(P<0.05),and no complications or adverse events occurred in both groups of patients.Conclusion The acupoint application of Zhengchang Powder effectively shortens the retention time of gastric tubes,helps blood potassium concentration maintain at normal levels,and improves gastrointestinal symptoms in patients with gastric cancer after surgery,with a significant therapeutic effect.Based on the theory of ascending and descending of stomach qi,this study discussed the application effect of the acupoint application of Zhengchang Powder and provided
