基于6S管理阶段护理应用在血液透析重症患者中对其护理依从性及并发症的影响

摘　　要：目的 探讨基于6S管理阶段护理在血液透析重症患者中的应用效果。方法 采用随机数表法将2021年9月至2022年9月于厦门大学附属第一医院重症室行血液透析治疗的96例重症患者分为对照组(48例,采取常规护理)和观察组(48例,采用基于6S管理阶段护理)。两组均持续护理3个月。比较两组护理依从性、肾功能、并发症、舒适度和生活质量。结果 观察组护理后护理依从性中合理饮食、规律运动、正确服药以及自我监测评分均高于对照组(P<0.05)。观察组护理后肌酐清除率(CCr)高于对照组,血尿素氮(BUN)、血清肌酐(SCr)均低于对照组(P<0.05)。观察组并发症总发生率为4.17%(2/48),低于对照组的16.67%(8/48)(P<0.05)。观察组护理后舒适状况量表(GCQ)中社会文化、生理、心理及环境评分高于对照组(P<0.05)。观察组护理后生活质量测定量表简表(QOL-BREF)中环境、生理、心理、社会评分均高于对照组(P<0.05)。结论 基于6S管理阶段护理可提升血液透析重症患者护理依从性,促进患者肾功能改善,降低导管感染、低血压等并发症发生风险,提升患者舒适度,从而提高患者生活质量。Objective To explore the application effect of nursing based on the 6S management stage in severe hemodialysis patients.Methods A total of 96 severe patients who underwent hemodialysis in the ICU of the First Affiliated Hospital of Xiamen University from September 2021 to September 2022 were divided into the control group(48 cases,with routine nursing)and the observation group(48 cases,with nursing based on 6S management stage)using random number table method.Both groups received continuous nursing for 3 months.The nursing compliance,renal function,complications,comfort level,and quality of life between the two groups were compared.Results The observation group showed higher scores in terms of nursing compliance,including a reasonable diet,regular exercise,correct medication administration,and self-monitoring compared to the control group(P<0.05).After nursing,the creatinine clearance rate(CCr)in the observation group was higher than that in the control group,while the blood urea nitrogen(BUN)and serum creatinine(SCr)were lower than those in the control group(P<0.05).The total incidence of complications in the observation group was 4.17%(2/48),lower than the 16.67%(8/48)in the control group(P<0.05).The observation group had higher scores in terms of social culture,physiology,psychology,and environment on the general comfort questionnaire(GCQ)compared to the control group(P<0.05).The environmental,physiological,psychological,and social scores in the quality of life brief scale of the observation group were higher than those in the control group(P<0.05).Conclusion Nursing based on the 6S management stage can improve the nursing compliance of severe hemodialysis patients,promote the improvement of renal function,reduce the risk of catheter infection,hypotension and other complications,improve the comfort level of patients,and thus improve the quality of life of patients.
