右美托咪定结合罗哌卡因在低位硬膜外麻醉中的应用及安全性研究

摘　　要：目的 分析右美托咪定结合罗哌卡因的麻醉方案在低位硬膜外麻醉中的应用效果及安全性。方法 选取2022年1—12月邳州市中医院接收的70例低位硬膜外麻醉患者为研究对象,采用随机数表法分成常规组和研究组,每组各35例。常规组单纯使用罗哌卡因麻醉,研究组以右美托咪定联合罗哌卡因麻醉,比较两组感觉阻滞起效时间、阻滞持续时间、运动阻滞起效与持续时间、不良反应、麻醉满意度、视觉模拟评分法(VAS)评分、Ramsay镇静评分。结果 研究组感觉阻滞起效时间短于常规组,但持续时间长于常规组,差异有统计学意义(P<0.05)。两组在运动阻滞起效与持续时间上比较,差异无统计学意义(P>0.05)。研究组VAS评分低于常规组,差异有统计学意义(P<0.05)。研究组Ramsay镇静评分高于常规组,差异有统计学意义(P<0.05)。研究组不良反应总发生率低于常规组,差异有统计学意义(P<0.05)。研究组麻醉满意度高于常规组,差异有统计学意义(P<0.05)。结论 选择右美托咪定结合罗哌卡因药物进行低位硬膜外麻醉所获得的麻醉效果较好,减少不良反应,具有一定安全性,同时还能在一定程度上减轻疼痛程度,值得推广。Objective To analyze the effect and safety of dexmedetomidine combined with ropivacaine in low epidural anesthesia.Methods The period from January to December 2022 was the time interval for the study,and 70 patients with low epidural anesthesia received by Pizhou Hospital of Traditional Chinese Medicine were selected as the main investigation objects.They were divided into two groups by the random number table method,namely,the conventional group and the study group,with 35 cases as the number of people in each group.The former used ropivacaine alone during anesthesia,while the latter used dexmedetomidine combined with ropivacaine.The onset time and duration of sensory block,motor block onset time and duration,adverse reactions,anesthesia satisfaction,visual analogue scale(VAS)score,and Ramsay sedation score were compared between the two groups.Results The onset time of sensory blockade in the study group was shorter than that in the conventional group,but the duration of block was longer than that in the conventional group,with a statistically significant difference(P<0.05).There was no statistically significant difference in the onset and duration of the motor block between the conventional group and the study group(P>0.05).The VAS score of the study group was lower than that of the conventional group,with a statistically significant difference(P<0.05).The Ramsay sedation score in the study group was higher than that in the conventional group,with a statistically significant difference(P<0.05).The incidence of adverse reactions in the study group was lower than that in the conventional group,with a statistically significant difference(P<0.05).The anesthesia satisfaction of the study group was higher than that of the conventional group,with a statistically significant difference(P<0.05).Conclusion The narcotic dexmedetomidine combined with ropivacaine drugs for low level epidural anesthesia has an ideal anesthetic effect,reduces the adverse reactions,and can reduce pain to a certain extent,which has certain safe
