摘 要:目的探讨应用钛网行胸廓重建与肋骨接骨板行胸廓重建的临床效果。方法选取2018年5月至2023年5月联勤保障部队第九八八医院心胸外科需行胸廓重建手术的患者30例,采用随机数表法将其分为两组,每组各15例。观察组患者应用钛网行胸廓重建,对照组患者应用肋骨接骨板行胸廓重建。比较两组手术及术后康复指标、手术前后视觉模拟评分法(VAS)评分、用力肺活量(FVC)、第1秒用力呼气容积(FEV_(1))、最大呼气峰流速(PEF)及并发症情况。结果观察组患者手术时间、骨折愈合时间及术中出血量少于对照组,差异有统计学意义(P<0.05)。术前及术后1、3、7d两组患者VAS评分比较,差异无统计学意义(P>0.05)。术后1个月观察组FVC、FEV_(1)、PEF高于对照组,差异均有统计学意义(P<0.05)。结论应用钛网与肋骨接骨板行胸廓固定均是胸廓重建术患者安全有效的治疗方式,可有效缓解患者术后疼痛,但前者能优化手术操作,改善肺功能,促进骨折愈合。Objective To explore the clinical effects of using titanium mesh for thoracic reconstruction and rib bone plate for thoracic reconstruction.Methods A total of 30 patients who underwent thoracic reconstruction surgery at No.988 Hospital of Joint Logistics Support Force of the Chinese People’s Liberation Army from May 2018 to May 2023 were selected.The patients were divided into two groups using a random number table method.Among them,15 patients in the observation group underwent thoracic reconstruction using titanium mesh,while 15 patients in the control group underwent thoracic reconstruction using rib bone plate.The preoperative and postoperative rehabilitation indicators,preoperative and postoperative visual analogue scale(VAS)scores,forced vital capacity(FVC),forced expiratory volume in one second(FEV_(1)),peak expiratory flow(PEF),and complications were compared between the two groups.Results The surgical time,fracture healing time,and intraoperative bleeding in the observation group were significantly less than those in the control group,with statistically significant differences(P<0.05).The VAS scores between the two groups of patients before surgery and 1 day,3 days,and 7 days after surgery were compared,without statistically significant differences(P>0.05).One month after surgery,the FVC,FEV_(1),and PEF levels in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion The applications of titanium mesh and rib bone plate for thoracic fixation are both safe and effective treatment methods for patients undergoing thoracic reconstruction surgery,which can effectively alleviate postoperative pain of patients.However,the former can optimize surgical procedures,improve lung function,and promote fracture healing.
