摘 要:目的:探究川崎病(Kawasaki disease,KD)患儿凝血功能、血脂水平与冠脉损伤(Coronary arterial lesions,CAL)的相关性。方法:回顾性分析2019年至2022年我院收治的60例KD患儿临床资料,并选取健康儿童60例为对照组,根据KD组CAL情况分为CAL组与NCAL组,对照组于体检当日、KD组分别于急性期与亚急性期采用凝血分析仪检测凝血功能:凝血酶原时间(Prothrombin time,PT)、凝血酶时间(Thrombin time,TT)、活化部分凝血活酶时间(Activated Partial Thromboplastin Time,APTT)、D-二聚体(D-Dimer,D-D);采用生化分析仪检测血脂:总胆固醇(Total Cholesterol,TC)、甘油三酯(Triglyceride,TG)、低密度脂蛋白(Low density lipoprotein,LDL)、高密度脂蛋白(High density lipoprotein,HDL)水平;KD组治疗1 m后比较CAL组与NCAL组凝血功能及血脂水平。结果:KD组PT、TT、APTT、HDL水平低于对照组,D-D、TG水平高于对照组(P<0.05);急性期PT、TT、APTT水平低于亚急性期,D-D水平高于亚急性期(P<0.05)。Spearman相关性显示CAL与PT、TT及HDL水平成负相关,与D-D、TG成正相关(P<0.05)。结论:KD患儿急性期高凝状态明显,PT、TT、HDL水平降低,D-D、TG水平升高会增加KD患儿CAL发生风险。Objective:The purpose of this study was to investigate the association of coagulation function and blood lipid levels with coronary artery lesions(CAL)in children with Kawasaki disease(KD).Methods:The clinical data of 60 children with KD admitted to the hospital from 2019 to 2022 were analyzed retrospectively.Sixty healthy children were selected as the control group.Children with KD were divided into the CAL group and the NCAL group.Coagulation function,including prothrombin time(PT),thrombin time(TT),activated partial thromboplastin time(APTT),and D-dimer(D-D)was detected using a blood coagulation analyzer,and blood lipids,such as total cholesterol(TC),triglyceride(TG),low-density lipoprotein(LDL)and high-density lipoprotein(HDL)were detected using a biochemical analyzer.Coagulation function and blood lipid levels were compared between the CAL and NCAL groups after 1 month of treatment.Results:PT,TT,APTT,and HDL levels in the KD group were shorter/lower than in the control group.The levels of D-D and TG were higher than those in the control group(P<0.05).PT,TT,and APTT were shorter in the acute phase than in the subacute phase,and D-D levels were higher in the acute phase(P<0.05).Spearman correlation analysis found that CAL was negatively correlated with PT,TT,and HDL levels,and positively correlated with D-D and TG levels(P<0.05).Conclusion:Children with KD have a significant hypercoagulable state in the acute phase.Decreased PT,TT,and HDL levels,and elevated D-D and TG levels will increase the risk of CAL in children with KD.
