血清S100A8/A9、MMP3、GPI在膝关节骨性关节炎中的变化及临床意义

摘　　要：目的:探究血清钙卫蛋白(Calprotectin)S100A8/A9、基质金属蛋白酶3(Matrix metalloproteinase3,MMP3)、葡萄糖-6-磷酸异构酶(Glucose-6-phosphate isomerase,GPI)指标在膝关节骨性关节炎中的诊断价值。方法:选取2020年1月至2022年9月我院收治的82例膝关节骨性关节炎患者作为研究对象的观察组,另选取43例同期健康体检者作为研究对象的对照组。检测并比较S100A8/A9、MMP3、GPI水平;并根据Kellgren-Lawrence(K-L)分级将病情分为轻中度组和重度组,对比不同病情严重程度患者S100A8/A9、MMP3、GPI水平,并采用接收者操作特征曲线(Receiver operating characteristic curve,ROC)曲线分析上述指标对重度膝关节骨性关节炎患者的预测价值。结果:观察组S100A8/A9、MMP3、GPI水平均明显高于对照组(P<0.05),重度患者S100A8/A9、MMP3、GPI水平高于对照组(P<0.05),经ROC曲线分析,S100A8/A9诊断膝关节骨性关节炎曲线下面积(Area Under Curve,AUC)为0.950,MMP3诊断疾病AUC为0.978、GPI诊断疾病AUC为0.861,S100A8/A9、MMP3灵敏度和特异度均较GPI高(P<0.05)。结论:S100A8/A9、MMP3、GPI对重度膝关节骨性关节炎疾病具有一定的预测价值。Objective:To investigate the serum Calprotectin S100A8/A9,matrix metalloproteinase 3(MMP3),glucose-6-phosphate isomerase and the diagnostic value of GPI in knee osteoarthritis.Methods:A total of 82 patients with knee osteoarthritis admitted to our hospital from January 2020 to September 2022 were selected as the observation group,and 43 healthy subjects in the same period were selected as the control group.S100A8/A9,MMP3,GPI levels were detected and compared.According to Kellgren-Lawrence(K-L)classification,the disease was divided into mild to moderate group and severe group,and S100A8/A9,MMP3 and GPI levels of patients with different severity of the disease were compared.Receiver operating characteristic curve(ROC)curve was used to analyze the predictive value of the above indexes in patients with severe knee osteoarthritis.Result:The levels of S100A8/A9,MMP3 and GPI in the observation group were significantly higher than those in the control group(P<0.05),and the levels of S100A8/A9,MMP3 and GPI in severe patients were higher than those in the control group(P<0.05).The Area Under Curve(AUC)of S100A8/A9 for diagnosis of knee osteoarthritis was 0.950,that of MMP3 for diagnosis of disease was 0.978,and that of GPI for diagnosis of disease was 0.861.The sensitivity and specificity of S100A8/A9 and MMP3 were higher than GPI(P<0.05).Conclusion:S100A8/A9,MMP3 and GPI have certain predictive value in severe knee osteoarthritis.
