摘 要:目的:探讨二维联合四维超声筛查不同孕周胎儿畸形的价值。方法:选取我院2020年12月至2021年5月接收的不同孕周疑似胎儿畸形的孕妇155例,均同时行二维和四维超声检查,以最终引产/分娩结果作为金标准,对比二维超声、四维超声以及联合检查的诊断价值。结果:155例疑似胎儿畸形孕妇中,引产/分娩结果为阳性(胎儿畸形)75例,阴性(胎儿正常)80例;二维联合四维超声的诊断敏感性(86.67%)高于单纯二维超声(69.33%)(P<0.05);二维联合四维超声孕早期畸形胎儿检出率(28.00%)高于单纯二维超声(12.00%)(P<0.05);随孕周增加,二维联合四维超声的诊断准确率逐渐增加,孕晚期(100.00%)>孕中期(83.33%)>孕早期(47.62%),差异有统计学意义(P<0.05)。结论:二维联合四维超声在不同孕周胎儿畸形中具有较高检出率,且孕晚期的诊断准确率最高。Objective:To explore the values of two-dimensional combined with four-dimensional ultrasound in screening fetal malformations at different gestational weeks.Methods:One hundred and fifty-five pregnant women with suspected fetal malformation at different gestational weeks who were received from our hospital from December 2020 to May 2021 were selected,and all of them underwent two-dimensional and four-dimensional ultrasound examinations at the same time.The final results of induction/delivery were used as the gold standard,and the diagnostic values of two-dimensional ultrasound,four-dimensional ultrasound,and the combined examination were compared.Results:Among 155 pregnant women with suspected fetal malformation,75 cases were positive(fetal malformation)and 80 cases were negative(fetal normal)in the results of induction/delivery,the diagnostic sensitivity of two-dimensional combined with four-dimensional ultrasound(86.67%)was higher than that of two-dimensional ultrasound alone(69.33%)(P<0.05),the detection rate of fetal malformations in early pregnancy by two-dimensional combined with four-dimensional ultrasound(28.00%)was higher than that by two-dimensional ultrasound alone(12.00%)(P<0.05).With the increase of gestational weeks,the diagnostic accuracy of two-dimensional combined with four-dimensional ultrasound gradually increased,with late pregnancy(100.00%)>middle pregnancy(83.33%)>early pregnancy(47.62%)(P<0.05).Conclusion:Two-dimensional combined with four-dimensional ultrasound has a high detection rate in fetal malformations of different gestational weeks,and the diagnostic accuracy of late pregnancy is the highest.
