摘 要:目的:研究哌拉西林他唑巴坦联合无创正压通气对慢性阻塞性肺疾病(COPD)并呼吸衰竭患者动脉血气分析、血氧饱和度和肺部炎症的影响。方法:选取2020年5月至2023年5月收治的70例COPD并呼吸衰竭患者,按随机数字表法分为研究组与对照组,两组各35例。对照组给予无创正压呼吸机辅助通气,研究组给予哌拉西林他唑巴坦静脉滴注联合无创正压呼吸机辅助通气。分别检测两组患者治疗前后的动脉血气分析[包括动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、pH值。]、血氧饱和度[包括平均血氧饱和度(MSpO_(2))、最低血氧饱和度(LSpO_(2))]及炎性因子[C-反应蛋白(CRP)、降钙素原(PCT)]水平及两组患者在治疗后的生存率。结果:两组患者在治疗后血气指标PaCO_(2)下降,PaO_(2)、pH值均上升,且研究组患者改善较对照组更显著(P<0.05);两组患者在治疗后MSpO_(2)、LSpO_(2)水平均上升,且研究组患者的上升程度显著高于对照组(P<0.05);炎性因子CRP、PCT水平都较治疗前下降,且研究组患者的水平较对照组更低(P<0.05);研究组患者生存率94.29%较对照组患者生率77.14%更高。结论:哌拉西林他唑巴坦联合无创正压通气对COPD并呼吸衰竭患者可有效改善患者的动脉血气指标、血氧饱和度指标及炎性因子水平,纠正患者的呼吸衰竭,改善预后。Objective:To study the effects of piperacillin and tazobactam combined with non-invasive positive pressure ventilation on arterial blood gas analysis,blood oxygen saturation and pulmonary inflammation in patients with chronic obstructive pulmonary disease(COPD)and respiratory failure.Methods:70 patients with COPD and respiratory failure admitted from May 2020 to May 2023 were divided into study group and control group according to random number table method,with 35 cases in each group.The control group was given non-invasive positive pressure ventilator assisted ventilation,and the study group was given piperacillin and tazobactam intravenous infusion combined with non-invasive positive pressure ventilator assisted ventilation.Arterial blood gas analysis[including PaO_(2),PaCO_(2) and pH values]was performed before and after treatment in both groups.,blood oxygen saturation(including mean oxygen saturation(MSpO_(2)),minimum oxygen saturation(LSpO_(2))),inflammatory factors[C-reactive protein(CRP),procalcitonin(PCT)]levels,and survival rates after treatment in both groups.Results:After treatment,blood gas index PaCO_(2) decreased,PaO_(2) and pH increased,and the improvement in the study group was more significant than that in the control group(P<0.05).The levels of MSpO_(2) and LSpO_(2) in both groups were increased after treatment,and the degree of increase in the study group was significantly higher than that in the control group(P<0.05).The levels of inflammatory factors CRP and PCT were decreased compared with those before treatment,and the levels in the study group were lower than those in the control group(P<0.05).The survival rate of the study group was 94.29%higher than that of the control group,which was 77.14%.Conclusion:Piperacillin and tazobactam combined with non-invasive positive pressure ventilation can effectively improve the levels of arterial blood gas indexes,blood oxygen saturation indexes and inflammatory factors in patients with COPD and respiratory failure,correct the respiratory failure of
