摘　　要：目的:探究BiPAP无创呼吸机联合氨溴索治疗重症肺炎的疗效及对炎症反应的影响。方法:回顾性收集医院急诊科于2020年1月-2022年7月收治的重症肺炎患者80例的临床资料。按照治疗方法的不同分为观察组和对照组,每组各40例。对照组采用氨溴索治疗,观察组采用氨溴索联合BiPAP无创呼吸机治疗。观察两组患者治疗效果、临床症状、炎症因子水平和血气指标。结果:观察组总有效率(92.50%)高于对照组(75.00%)(P<0.05)。观察组退烧、咳嗽消失、胸痛消失与肺部啰音消失时间短于对照组(P<0.05)。观察组的白介素-6(IL-6)、C反应蛋白(CRP)、降钙素原(PCT)、急性生理与慢性健康状况(APACHEⅡ)评分均低于对照组(P<0.05)。观察组患者血气指标氧合指数(OI)、血氧饱和度(SaO_(2))、动脉氧分压(PaO_(2))优于对照组(P<0.05)。结论:对于重症肺炎的治疗,BiPAP无创呼吸机联用氨溴索具有良好的效果,炎症反应及血气指标改善效果显著。Objective:To explore the efficacy of BiPAP noninvasive ventilator combined with ambroxol in the treatment of severe pneumonia and its effect on inflammation.Methods:The clinical data of 80 patients with severe pneumonia admitted to the emergency department of the hospital from January 2020 to July 2022 were retrospectively collected.According to the different treatment methods,they were divided into observation group and control group,40 cases in each group.The control group was treated with ambroxol,and the observation group was treated with ambroxol combined with BiPAP noninvasive ventilator.The therapeutic effect,clinical symptoms,levels of inflammatory factors and blood gas indexes of the two groups were observed.Results:The total effective rate of observation group(92.50%)was higher than that of control group(75.00%)(P<0.05).The time of fever,cough,chest pain and lung rales disappearing in observation group was shorter than that in control group(P<0.05).The scores of interleukin-6(IL-6),C-reactive protein(CRP),procalcitonin(PCT),acute physiological and chronic health status(APACHEⅡ)were all lower than those of control group(P<0.05).Blood gas oxygenation index(OI),blood oxygen saturation(SaO_(2))and arterial oxygen partial pressure(PaO_(2))in observation group were better than those in control group(P<0.05).Conclusion:For the treatment of severe pneumonia,BiPAP noninvasive ventilator combined with ambroxol has a good effect,and the inflammatory response and blood gas indexes are significantly improved.
