摘 要:皮肤撕脱伤形成复杂而难以处理的创面,临床上多采用清创及有活力的撕脱皮瓣原位缝合,同时借助其他辅助治疗逐步修复创面。皮肤撕脱伤常继发感染,处理策略需依据损伤部位及撕脱皮瓣的活力而决定。然而,目前尚缺乏可靠的方法早期准确预测撕脱皮瓣活力及界定其坏死范围。分子影像学技术提供了新的诊断方向,相关技术也将转化为临床应用。本文旨在探讨皮肤撕脱伤撕脱皮瓣组织坏死机理,总结撕脱皮瓣活力早期评估与修复的研究进展,为临床诊治提供参考。Skin avulsion injury forms a complex and difficult wound.Clinically,debridement and dynamic avulsion flap are used in situ,and the wound is gradually repaired with the help of other auxiliary treatments.Skin avulsion is often secondary to infection,and the treatment strategy is based on the injury site and the vitality of the avulsed flap.However,reliable methods for early and accurate prediction of avulsion flap vitality and defining the scope of necrosis are lacking.Molecular imaging techniques offer new diagnostic directions,and the technologies will translate into clinical applications.This paper discusses the mechanism of tissue necrosis of skin avulsion flap,summarizes the research progress in early vitality assessment and repair of avulsion flap,and provides a reference for clinical diagnosis and treatment.
