摘　　要：目的对1例隐翅虫毒液沾染后发生隐翅虫重症皮炎合并下肢血管炎、湿疹性皮炎和细菌性感染的病例进行调查分析,为正确有效防治隐翅虫蜇伤提供借鉴和参考。方法采用流行病学调查、回顾性综合分析研讨方法对病例展开调查,收集其诊疗前后的临床资料,围绕隐翅虫重症皮炎病例的处理和转归,对其发生、治疗及结果进行全面梳理、归纳、分析和总结。结果通过对病例开展个案调查和综合研判,该病例系沾染了毒隐翅虫毒液所致的急性隐翅虫皮炎所致,治疗期间继发的多重不良反应考虑与病情延误、自身免疫状态和病例的一系列错误操作等多因素相关,如继发下肢血管炎可能与病例使用75%酒精喷雾相关,湿疹性皮炎则与其使用硫酸镁溶液热敷引起的感染有关,下肢感染考虑其乱用药物及徒手抓破脓疱导致双下肢继发细菌感染。结论隐翅虫皮炎是一种好发于夏秋季节的常见疾病,临床报道较少,不及时治疗,可引起重症皮炎及多重不良反应,应加强宣教和在高发时期加强防范意识。Objective To investigate and analyze a case of Paederus severe dermatitis combined with lower extremity vasculitis,eczematous dermatitis and bacterial infection after Paederus venom contamination,so as to provide lessons and references for the correct and effective prevention and treatment of Paederus stings.Methods Epidemiological investigation and retrospective comprehensive analysis seminar methods were used to investigate the case.Clinical data were collected before and after the case diagnosis and treatment,and the occurrence,treatment and outcome around the management and regression after the occurrence of Paederus severe dermatitis of the cases were comprehensively sorted out,summarized,analyzed and summarized.Results Through case investigation and comprehensive research and judgment of the case,the case was caused by acute Paederus dermatitis due to contact with the venom of Paederus.Multiple adverse reactions secondary to the treatment period were considered to be related to multiple factors such as delayed in illness,autoimmune status,and a series of misoperations in the cases,such as secondary lower extremity vasculitis possibly related to the use of 75%alcohol spray,eczematous dermatitis related to an infection caused by the use of hot compress with magnesium sulfate solution.The lower extremity infection in this case was considered to be a secondary bacterial infection of both lower extremities due to indiscriminate use of medication and barehanded scratching to break pustules.Conclusion Paederus dermatitis is a common disease that tended to occur in summer and autumn,with fewer clinical reports,and without timely treatment,it causes severe dermatitis and multiple adverse reactions,and education and prevention awareness should be strengthened during the high incidence period.
