摘 要:目的探讨Exendin-4对兔自体游离脂肪移植保留率的影响。方法自2020年9月至2022年3月,哈尔滨医科大学附属第二医院整形美容科将兔随机均分为3组即对照组(生理盐水组)、实验组A(10 nmol/L Exendin-4)和实验组B(20 nmol/L Exendin-4),建立兔背部自体脂肪移植模型。分别在术后2、4、12周对移植物进行取材,观察移植物的大体保留情况,通过天平及排水法测定移植物的质量及体积。并对移植物行HE染色观察组织纤维化、炎症浸润、细胞完整性等情况;CD31免疫组织化学染色,观察移植物的血管生成情况。结果实验组A与实验组B的脂肪移植物的质量和体积明显高于对照组(P<0.05)。在不同时间点,与对照组相比,实验组A与实验组B脂肪移植物具有较少的纤维结缔组织、炎症细胞的浸润及空泡出现情况均较少(P<0.05)。随着实验时间的不断延长,各组移植物的血管密度均有所增加。在各时间点实验组A与实验组B均较对照组具有更好的血管生成情况(P<0.05)。与实验组A比较,实验组B具有更好的移植物体积保留率以及更好的脂肪细胞完整性,血管的生成情况也更佳。结论Exendin-4可通过减轻移植脂肪组织的炎症浸润、减少移植物的纤维化程度、减少囊泡的形成以及促进移植物血管生成提高脂肪移植物的体积保留率。Objective Toinvestigate the effect of Exendin-4 on the survival ofautologous free fat transplantation.Methods According to the different reagents added,rabbits were randomly divided into three groups:control group(normal saline group),experimental group A(10 nmol/L Exendin-4)or experimental group B(20 nmol/L Exendin-4),and the back autologous fat transplantation model was established.At 2,4 and 12 weeks after operation,the grafts were sampled,the general survival of the grafts was observed,and the mass and volume of the grafts were measured by balance and drainage method.After that,HE staining was performed to observe tissue fibrosis,inflammatory infiltration and cell integrity.The angiogenesis of the grafts was observed by CD31 immunohistochemical staining.Results The weight and volume of fat grafts in the experimental group were significantly higher than those in the control group(P<0.05).At different time points,compared with the control group,the experimental group had less fibrous connective tissue,less inflammatory cell infiltration and less vacuole(P<0.05).With the continuous extension of the experiment time,the vascular density of the grafts in all groups increased.Both experimental group A and experimental group B had better angiogenesis than control group at each time point(P<0.05).Compared with experimental group A,experimental group B had better graft volume retention,better adipocyte integrity,and better angiogenesis.Conclusion Exendin-4 can improve the survival rate of fat grafts by reducing the inflammatory infiltration of transplanted fat tissue,reducing the degree of fibrosis,reducing the formation of vesicles and promoting graft angiogenesis.
