摘  要:目的他汀类药物已被推荐用于缺血伴非阻塞性冠状动脉粥样硬化性心脏病(ischemia and non-obstructive coronary artery disease,INOCA)患者二级预防,本文研究阿托伐他汀治疗对INOCA患者外周血基质细胞衍生因子-1(stromal cell derived factor-1,SDF-1)和血管内皮生长因子(vascular endothelial growth factor,VEGF)短期内的影响。方法选择2020年1月至2020年12月上海市杨浦区市东医院冠状动脉计算机断层扫描血管造影(computed tomography angiography,CTA)或冠状动脉造影提示冠状动脉血管正常或冠状动脉血管狭窄小于50%的100例患者,分成危险因素控制组(Risk组)(50例)和INOCA组(50例),检测阿托伐他汀治疗前后SDF-1和VEGF浓度的变化。结果与危险因素控制组患者相比,INOCA组患者的VEGF浓度差异无统计学意义(t=-0.252,P=0.801);SDF-1浓度较低,差异有统计学意义(t=2.103,P=0.039)。阿托伐他汀治疗后,INOCA组患者的VEGF和SDF-1浓度显著升高,差异有统计学意义(t=-3.850,P=0.000;t=-2.313,P=0.025),且治疗后两者表达呈正相关(r=0.415,P=0.003)。结论阿托伐他汀治疗INOCA患者可使VEGF与SDF-1浓度短期内升高,进而协同促进血管增生,改善INOCA患者预后。Objectives Statins have been recommended in secondary prevention for patients with ischemia and nonobstructive coronary artery diseasee(INOCA).Concentrations of stromal cell derived factor-1(SDF-1)and vascular endothelial growth factor(VEGF)were detected to study the short-term effect of moderate-dose atorvastatin treatment on patients with INOCA.Methods Totally 100 patients with coronary artery stenosis less than 50%examined by com⁃puted tomography angiography(CTA)or coronary angiography were included from January 2020 to December 2020 in Shidong Hospital.The patients were divided into Risk group(50 cases)and INOCA group(50 cases).Concentrations of SDF-1 and VEGF were detected before and after the application of atorvastatin.Results Compared with Risk group,there was no significant difference in VEGF concentrations(t=-0.252,P=0.801),but the difference of SDF-1 was statisti⁃cally significant(t=2.103,P=0.039)in INOCA group.After atorvastatin treatment for half a year,the concentrations of VEGF and SDF-1 increased significantly(t=-3.850,P=0.000;t=-2.313,P=0.025),and which were positively correlated(r=0.415,P=0.003)in INOCA group.Conclusions Atorvastatin treatment can increase the concentrations of VEGF and SDF-1 in a short term,which may promote vascular proliferation and improve the prognosis of patients with INOCA.
