摘　　要：目的:探究负压封闭引流技术(Vacuum sealing drainage,VSD)在自体皮片移植治疗下肢深度烧伤创面中的效果。方法:选择2018年8月-2021年7月笔者医院收治的100例下肢深度烧伤患者,按随机数字表法分为对照组和研究组各50例。对照组给予自体皮片移植结合传统换药治疗,研究组患者给予自体皮片移植结合VSD治疗。比较两组创面愈合质量、疼痛情况、换药次数、创面愈合时间、住院时间、并发症情况、温哥华瘢痕评分及患者满意度。结果:术后,研究组创面愈合优良率高于对照组(P<0.05),换药次数少于对照组(P<0.05),创面愈合时间及住院时间短于对照组(P<0.05),并发症发生率低于对照组(P<0.05);术后14 d,研究组植皮成活率高于对照组(P<0.05),疼痛评分低于对照组(P<0.05);术后12个月,研究组VSS评分低于对照组(P<0.05),患者满意度高于对照组(P<0.05)。结论:VSD联合自体皮片移植治疗下肢深度烧伤创面疗效显著,患者创面愈合良好、疼痛轻、并发症少、美观度好、满意率高,值得临床推广。Objective To explore the cosmetic effect of vacuum sealing drainage(VSD)combined with autologous skin grating in the treatment of lower limb deep burn wounds.Methods A total of 100 patients with deep burns of lower limbs admitted to the author's hospital from August 2018 to July 2021 were selected and divided into a control group and a study group according to the random number table method,with 50 cases in each group.The control group was treated with autologous skin grafting combined with traditional dressing change,and the study group was treated with autologous skin grafting combined with VSD.The wound healing quality,pain,dressing change times,wound healing time,length of hospital stay,complications,Vancouver scar score and patient satisfaction were compared between the two groups.Results After operation,the excellent rate of wound healing in the study group was higher than that in the control group(P<0.05),the number of dressing change was less than that in the control group(P<0.05),the wound healing time and hospitalization time were shorter than those in the control group(P<0.05),and the incidence of complications was lower than that in the control group(P<0.05).At 14 days after operation,the survival rate of skin graft in the study group was higher than that in the control group(P<0.05),and the pain score was lower than that in the control group(P<0.05).At 12 months after operation,the VSS score of the study group was lower than that of the control group(P<0.05),and the patient satisfaction was higher than that of the control group(P<0.05).Conclusion VSD combined with autologous skin grafting is effective in the treatment of deep burn wounds of lower limbs,with good wound healing,light pain,few complications,good aesthetics,and high satisfaction rate,which is worthy of clinical promotion.
