六神丸外敷联合红蓝光及米诺环素治疗重度痤疮的临床观察

摘　　要：目的:观察六神丸外敷联合红蓝光治疗仪照射、口服米诺环素治疗重度痤疮疗效及对白介素-8(IL-8)、肿瘤坏死因子(TNF-α)水平的影响。方法:选取笔者医院2020年11月-2022年4月收治的120例重度痤疮患者作为研究对象,采用随机数字法分为联合组和对照组,每组60例。对照组给予红蓝光治疗仪照射及盐酸米诺环素口服治疗,联合组在对照组基础上给予六神丸外敷联合盐酸米诺环素口服治疗,两组均连续治疗8周。观察两组临床疗效,比较治疗前后两组患者皮损严重程度评分(Global acne grading system,GAGS)、皮肤屏障功能(角质层含水量、皮脂分泌量)、皮肤美观度(皮肤弹性指数、血红素含量、黑色素指数)、两组血清白介素-8(Interleukin-8,IL-8)、肿瘤坏死因子(Tumor necrosis factor factor,TNF-α)、可溶性白细胞介素-2受体(Soluble interleukin-2receptor,sIL-2R)、干扰素(Interferon-γ,IFN-γ)水平及不良反应情况。结果:联合组治疗总有效率为95.00%,明显高于对照组的83.33%(P<0.05);治疗后,两组患者的GAGS评分均呈逐渐降低趋势,组内不同时间点比较差异有统计学意义(P<0.05),且联合组治疗后各时间点GAGS评分显著低于对照组(P<0.05);治疗8周后,两组皮肤角质层含水量显著高于治疗前(P<0.05),皮脂分泌量显著低于治疗前(P<0.05),皮肤弹性指数显著高于治疗前(P<0.05),血红素含量、黑色素指数显著低于治疗前(P<0.05),血清TNF-α、IL-8、sIL-2R水平均显著低于治疗前(P<0.05),血清IFN-γ水平显著高于治疗前(P<0.05),且联合组改善均优于对照组(P<0.05);联合组不良反应发生率为8.33%,对照组为6.67%,两组间比较差异无统计学意义(P>0.05)。结论:六神丸外敷联合红蓝光及米诺环素治疗重度痤疮疗效良好,可有效改善患者皮损、皮肤屏障功能及美观度,抑制炎症反应且安全性良好,值得推广。Objective To observe the efficacy of external application of Liushen pill combined with red-blue light and minocycline in treating severe acne and its effects on levels of interleukin-8(IL-8)and tumor necrosis factor(TNF-α).Methods 120 patients with severe acne who were treated in the second hospital of Baoding City from November 2020 to April 2022 were selected as the research subjects,and they were divided into combined group(60 cases)and control group(60 cases)by random number table method.The control group was given red-blue light irradiation and oral administration of minocycline hydrochloride while the combined group adopted external application of Liushen pill combined with oral administration of minocycline hydrochloride on the basis of the control group.Both groups were continuously treated for 8 weeks.The clinical efficacy of the two groups was observed and compared,and the skin lesion severity score(Global Acne Grading System,GAGS),skin barrier function(water content of stratum corneum,sebum secretion),skin aesthetics(skin elasticity index,heme content,melanin index),serum inflammatory indicators[IL-8,TNF-α,soluble interleukin-2 receptor(sIL-2R),interferon-γ(IFN-γ)]and adverse reactions were compared between the two groups before and after treatment.Results The total effective rate of treatment in combined group was 95.00%,which was significantly higher than 83.33%in control group(P<0.05).After 4 weeks and 8 weeks of treatment,the GAGS score of the two groups showed a gradually decreasing trend,and the difference was significant at different time points within the groups(P<0.05),and the GAGS score of combined group at each time point after treatment was significantly lower than that of control group(P<0.05).After 8 weeks of treatment,the water content of stratum corneum in the two groups was significantly higher than that before treatment(P<0.05)while the sebum secretion was significantly lower than that before treatment(P<0.05),and the skin elasticity index was significantly higher(P<0.05)wh
