摘  要:目的探讨核苷酸焦磷酸酶15(NUDT15)rs116855232基因多态性与6-巯基嘌呤(6-MP)致急性淋巴细胞白血病(ALL)患儿淋巴细胞降低的相关性。方法收集71例采用中国儿童肿瘤专业委员会ALL协作组2015(CCCG-ALL2015)方案或CCCG-ALL2020方案且完成环磷酰胺+阿糖胞苷+6-MP(CAT)阶段化疗的0~14岁ALL患儿的一般临床资料,采用单核苷酸多态性分析方法检测患儿的NUDT15基因,并分为野生(CC)型组和变异(CT+TT)型组,抽取化疗前及化疗期间患儿清晨空腹外周血,采用全自动血细胞分析仪观察不同NUDT15基因型患儿的白细胞(WBC)、中性粒细胞(ANC)、淋巴细胞、血红蛋白及血小板下降的最低值、下降速率及下降幅度等指标。结果CC型组与(CT+TT)型组患儿的淋巴细胞下降幅度和淋巴细胞下降速率比较,差异有统计学意义(P<0.05),WBC、ANC、血红蛋白、血小板下降的最低值、下降幅度及下降速率比较,差异均无统计学意义(P>0.05)。结论NUDT15 rs116855232基因多态性可能与ALL患儿使用6-MP后淋巴细胞降低有关。Objective To investigate the correlation between nucleotide pyrophosphatase 15(NUDT15)rs116855232 gene polymorphism and 6-mercaptopurine(6-MP)induced lymphocyte reduction in children with acute lymphoblastic leukemia(ALL).Methods Collecting general clinical data of 71 ALL child patients aged 0 to 14 who received Chinese children's cancer group acute lymphoblastic leukemia-2015(CCCG-ALL2015)or CCCG-ALL2020 regimen and completed cyclophosphamide+cytarabine+6-MP(CAT)stage chemotherapy.Collecting fasting peripheral blood from the child patients before and during chemotherapy in the morning,using a fully automated blood cell analyzer to detect blood routine indicators,and recording the lowest values,rate of descent and margin of decrease of white blood cells(WBC),neutrophils(ANC),lymphocytes,hemoglobin,and platelet count decline.Using single nucleotide polymorphism analysis to detect NUDT15 gene and classify as wild-type(CC)group and variant(CT+TT)group.Results There was a statistically significant difference between the CC type group and the(CT+TT)type group concerning rate of descent and margin of decrease of lymphocyte decline(P<0.05),but there was no statistically significant difference concerning lowest values,rate of descent and margin of decrease in WBC,ANC,hemoglobin,and platelets(P>0.05).Conclusions The NUDT15 rs116855232 gene polymorphism may be associated with decreased lymphocytes after 6-MP use in child patients with ALL.
