乳腺黏液癌患者超声图像特征与PR、ER、HER-2、ER、Ki-67的相关性

摘　　要：目的探讨乳腺黏液癌患者肿块超声图像特征与孕激素受体(PR)、雌激素受体(ER)、人表皮生长因子受体2(HER-2)、细胞增殖抗原标记物(Ki-67)的相关性。方法选取102例手术切除乳腺肿块且病理确诊乳腺黏液癌的患者为研究对象,采用彩色多普勒超声诊断仪检测患者肿块的超声图像特征(肿块部位、大小、形态、边界、内部回声、钙化灶、血流信号、纵横比及腋窝淋巴结转移情况)并进行乳腺影像报告和数据系统(BI-RADS)分级评估;取所有患者切除的乳腺黏液癌肿块,制作切片,采用免疫组织化学(IHC)法检测组织中PR、ER、HER-2及Ki-67的表达;采用Phi系数(φ)检验和非条件二分类logistic回归分析乳腺黏液癌患者超声图像特征与PR、ER、HER-2、Ki-67表达的关系。结果乳腺黏液癌组织中ER、HER-2、Ki-67表达与BI-RADS分级3~5类差异有统计学意义(P<0.05),PR表达与BI-RADS分级3~5类差异无统计学意义(P>0.05);患者淋巴结转移与PR阳性表达有相关性(P<0.05),肿块回声、纵横比、淋巴结转移与ER阳性表达有相关性(P<0.05),肿块回声、囊性变、血流、淋巴结转移与HER-2阳性表达有相关性(P<0.05),患者超声图像与Ki-67高表达无相关性(P>0.05);非条件二分类logistic回归分析结果表明,淋巴结转移是影响PR、ER阳性表达的危险因素(P<0.05)。结论乳腺黏液癌患者超声图像特征与PR、ER、HER-2、Ki-67表达存在一定相关性。Objective To investigate the characteristics of ultrasound images and progesterone receptors(PR),estrogen receptors(ER),human epidermal growth factor receptor-2(HER-2)and cell proliferation antigen marker(Ki-67)in patients with mucinous breast carcinoma(MBC).Methods A total of 102 pathologically confirmed MBC patients with surgically removed breast mass were selected as the subjects.The ultrasound image features(mass site,size,shape,boundary,internal echo,calcification foci,blood flow signal,aspect ratio and axillary lymph node metastasis)of the patients were detected by color Doppler ultrasonography,and the breast imaging report and Data System(BI-RADS)grading evaluation was performed.The breast myxoma excised from all patients was sliced and the expressions of PR,ER,HER-2 and Ki-67 in the tissues were detected by immunohistochemistry(IHC).The Phi coefficient(φ)was calculated by the four-cell table association analysis to examine the correlation between the ultrasonic image features of patients with MBC and PR,ER,HER-2,and Ki-67.Results There were statistically significant differences in the expression of ER,HER-2,and Ki-67 with grade 3 to 5 of BI-RADS(P<0.05),but no statistically significant differences in the expression of PR with grade 3 to 5 of BI-RADS(P>0.05).Lymph node metastasis was correlated with positive expression of PR(P<0.05);tumor echo,aspect ratio,lymph node metastasis were correlated with positive expression of ER(P<0.05);tumor echo,cystic change,blood flow,lymph node metastasis were correlated with positive expression of HER-2(P<0.05).There was no correlation between ultrasound images and Ki-67 high expression(P>0.05).Unconditioned binary logistic regression multivariate analysis showed that lymph node metastasis was a risk factor for PR and ER positive expression.Conclusions There is a certain correlation between the ultrasonic image features of patients with MBC and the expression of PR,ER,HER-2,and Ki-67,which can provide important reference value for the diagnosis,treatment and prognosis ev
