摘 要:目的:探讨不同类型先天性耳畸形的构成比例及高分辨率CT(HRCT)和磁共振成像(MRI)对先天性耳畸形的诊断价值。方法:回顾性分析2020年1月至2023年1月期间,在我院影像科诊断为先天性耳畸形患者的影像学资料,共74人,98耳。其中,69例行HRCT扫描,6例行MRI扫描。分析各种先天性耳畸形的构成比例并探讨HRCT和MRI对耳畸形的诊断价值。结果:74例先天性耳畸形病例中,单纯外耳畸形11例;单纯中耳畸形7例;外耳畸形合并中耳畸形16例;外耳畸形合并内耳畸形1例;中耳畸形合并内耳畸形2例;外耳、中耳及内耳混合畸形2例。结论:外耳、中耳畸形常同时发生,内耳畸形伴发外耳或中耳畸形较少。HRCT对骨质结构显示清晰,是目前先天性耳畸形的首选检查方法,MRI可用于对膜迷路和神经结构的显示,HRCT和MRI结合能清晰显示耳的解剖结构,是耳蜗植入术前必要的影像学检查。Objective:To investigate the proportion of different types of congenital ear malformations and the diagnostic value of high resolution computerized tomography(HRCT)and magnetic resonance imaging(MRI)in congenital ear malformations.Methods:From January 2020 to January 2023,the imaging data of 74 patients with a total of 98 ears diagnosed with congenital ear deformities in the Radiology Department of Tianjin First Central Hospital were collected for a retrospective study,of which 69 patients underwent HRCT scans,6 routine MRI scans.To analyze the proportion of various congenital ear malformations and to evaluate the diagnostic value of HRCT and MRI in ear malformations.Results:Among the74 patients with congenital ear malformations in this study,11 had external ear malformation alone;7 had simple middle ear malformation;16 had external ear malformation combined with middle ear malformation;1 had external ear malformation combined with inner ear malformation;2 cases of deformity;2 cases of mixed deformity of outer ear,middle ear and inner ear.Conclusion:Deformities of the outer ear and middle ear often occur at the same time,while inner ear deformity is rarely associated with outer ear or middle ear deformity.HRCT can clearly display the bone structure and is the preferred method for congenital ear deformities.MRI can be used to display the membranous labyrinth and nerve structure.The combination of HRCT and MRI can clearly display the anatomical structure of the ear and is a necessary imaging examination before cochlear implantation.
