摘  要:目的探讨miR-155对支气管哮喘患儿Th1/Th2细胞失衡的影响及致病机制。方法选取2018年7月至2022年6月在深圳市前海蛇口自贸区医院就诊的40例支气管哮喘发作期患儿为哮喘组,同期本院健康体检的30名儿童为对照组,测定并比较两组对象外周血Th1和Th2细胞亚群以及细胞因子(包括IL-4、IL-5、IFN-γ、IL-1、IL-17)水平。分离哮喘组、对照组外周血CD4^(+)T细胞,设为哮喘(细胞)组、对照(细胞)组;另用miR-155模拟物处理对照组分离获得的CD4^(+)T细胞,设为miR-155高表达组。检测并比较3组CD4^(+)T细胞miR-155、GATA结合蛋白3(GATA-3)、IL-4、IL-5表达水平。结果哮喘组外周血Th1比例、Th1/Th2以及IFN-γ水平均明显低于对照组(均P<0.05),Th2比例以及IL-4、IL-5、IL-1、IL-17水平均明显高于对照组(均P<0.05)。3组CD4^(+)T细胞miR-155、GATA-3、IL-4、IL-5表达水平比较差异均有统计学意义(均P<0.05),其中哮喘(细胞)组、miR-155高表达组CD4^(+)T细胞miR-155、GATA-3、IL-4、IL-5表达水平均明显高于对照(细胞)组(均P<0.05)。结论miR-155异常高表达在儿童支气管哮喘发病中发挥重要作用,其机制可能与上调GATA-3表达进而造成Th1/Th2细胞失衡有关。Objective To investigate the effect of microrNA(miR)-155 on T helper cell(Th)1/Th2 cell imbalance in children with bronchial asthma and its mechanism.Methods Forty children with bronchial asthma attack treated in Shenzhen Qianhai Shekou Free Trade Zone Hospital from July 2018 to June 2022 were selected as asthma group.In the same period,30 healthy children received health examination in the hospital were selected as the control group.The levels of Th1 and Th2 cell subsets and cytokines(including IL-4,IL-5,IFN-γ,IL-1,IL-17)in peripheral blood of the two groups were measured and compared.Peripheral blood CD4^(+)T cells of asthma group and control group were separated and divided into asthma(cell)group and control(cell)group.In addition,CD4^(+)T cells isolated from the control group were treated with miR-155 mimics and set as the miR-155 high expression group.The expression levels of miR-155,GATA-binding protein 3(GATA-3),IL-4 and IL-5 in CD4^(+)T cells of the three groups were detected and compared.Results The proportion of Th1,Th1/Th2 and IFN-γlevels in peripheral blood of asthma group were significantly lower than those of the control group(all P<0.05),and the proportion of Th2,and IL-4,IL-5,IL-1 and IL-17 levels were significantly higher than those of the control group(all P<0.05).There were significant differences in the levels of miR-155,GATA-3,IL-4 and IL-5 in CD4^(+)T cells among the 3 groups(all P<0.05),with those in asthma(cell)group and miR-155 high expression group significantly higher when compared with the control(cell)group(all P<0.05).Conclusion The abnormally high expression of miR-155 plays an important role in the pathogenesis of children bronchial asthma,and its mechanism may be related to the up-regulation of GATA-3 expression and further the imbalance of Th1/Th2 cells.
