摘 要:目的分析急性前交叉韧带(ACL)撕裂不伴半月板损伤患者膝关节骨性解剖特征与胫骨前移间的相关性。方法回顾性分析2020年1月至2021年12月中国人民武装警察部队海警总队医院收治的急性ACL撕裂不伴半月板损伤患者86例的临床资料,根据ACL损伤情况将患者分为完全撕裂组56例和部分损伤组30例。所有患者均行MRI检查,测量膝关节骨性解剖参数,分析膝关节骨性解剖特征与胫骨前移间的相关性。结果Pearson相关分析显示,胫骨前移与股骨外侧髁长度、胫骨外侧平台屈膝比均无相关性(r=-0.05、-0.15,均P>0.05),与股骨外侧髁高度、胫骨外侧平台长度均呈负相关(r=-0.81、-0.73,均P<0.01),与胫骨平台后倾角、外侧髁长高比、胫骨外侧平台伸膝比均呈正相关(r=0.77、0.87、0.82,均P<0.01)。两组患者各项膝关节骨性解剖特征比较,差异均无统计学意义(均P>0.05)。Pearson相关分析显示,完全损伤组中,胫骨前移与股骨外侧髁长度、胫骨外侧平台屈膝比均无相关性(r=-0.11、-0.39,均P>0.05),与股骨外侧髁高度、胫骨外侧平台长度均呈负相关(r=-0.85、-0.77,均P<0.01),与胫骨平台后倾角、外侧髁长高比、胫骨外侧平台伸膝比均呈正相关(r=0.74、0.85、0.82,均P<0.01)。部分损伤组中,胫骨前移与各项膝关节骨性解剖特征均无相关性(r=-0.42、-0.13、-0.35、0.07、0.17、0.33、-0.24,均P>0.05)。结论股骨外侧髁高度过低、胫骨外侧平台长度过小以及股骨外侧髁长度与股骨外侧髁高度、胫骨外侧平台长度的比值过大与胫骨前移增加相关,上述情况在ACL完全损伤时表现得更为明显。Objective To analyze the correlation between knee anatomical features and tibial anterior displacement in patients with acute anterior cruciate ligament(ACL)tear without meniscus injury.Methods The clinical data of 86 patients with acute ACL tear without meniscus injury treated in China Coast Guard Hospital of the People's Armed Police Force from January 2020 to December 2021 were retrospectively analyzed,and the 86 patients were divided into complete tear group(56 cases)and partial injury group(30 cases)according to different ACL injury conditions.Magnetic resonance imaging(MRI)was performed in all patients to measure the anatomical parameters of the knee joint,and to analyze the correlation between knee anatomical features and tibial anterior displacement.Results The results of Pearson correlation analysis showed that there was no correlation between tibial anterior displacement and the length of lateral femoral condyle and lateral tibial plateau flexion ratio(r=-0.05,-0.15,both P>0.05),but the tibial anterior displacement was negatively correlated with the height of lateral femoral condyle and length of lateral tibial plateau(r=-0.81,-0.73,both P<0.01)and positively correlated with the posterior inclination angle of the tibial plateau,the length-to-height ratio of the lateral condyle,and the extensor-knee ratio of the lateral tibial plateau(r=0.77,0.87,0.82,all P<0.01).There was no significant difference in the knee anatomical features in patients of the two groups(all P>0.05).Pearson correlation analysis showed that in the complete tear group,the tibial anterior displacement was not related to the length of the lateral femoral condyle and the knee flexion ratio of the lateral tibial plateau(r=-0.11,-0.39,both P>0.05),but it was negatively correlated with the height of the lateral femoral condyle and the lateral tibial plateau length(r=-0.85,-0.77,both P<0.01),while positively correlated with the posterior inclination angle of the tibial plateau,the aspect ratio of the lateral condyle,and the extensor-knee rat
