摘　　要：目的 了解住院费用及其结构变动情况,为评价控费效果、推进控费改革提供参考依据。方法 收集2018年1月—2022年12月南京和扬州市各1所传染病医院全部住院患者的病案首页信息,采用t检验、方差分析评估控费效果。结果样本医院的平均住院日远高于全国平均水平(t=167.913,P<0.001;t=92.579,P <0.001);A医院2021年住院费用高于2018—2020年,2022年住院费用低于2018—2021年(F=243.217,P <0.001);B医院5年住院费用差异无统计学意义(F=138.149,P=0.076)。结论 样本医院平均住院日有很大缩短空间,A医院2018—2021年控费不及预期,DRG实施后控费成效明显,B医院住院费用结构稳定、趋于优化。Objective To understand the hospitalization expenses and structural changes,and to provide reference for evaluating the effectiveness of cost control,promoting cost control reform.Methods The homepage information of all inpatients from one infectious disease hospital in Nanjing and Yangzhou from January 2018 to December 2022 were collected,and the cost control effect using T-test and analysis of variance were evaluated.Results The average hospitalization days of the sample hospitals were much higher than the national average(t=167.913,P<0.001;t=92.579,P<0.001).The hospitalization expenses of Hospital A in 2021 were higher than those from 2018 to 2020,while the hospitalization expenses in 2022 were lower than those from 2018 to 2021(F=243.217,P<0.001).There was no significant difference in the 5-year hospitalization expenses of Hospital B(F=138.149,P=0.076).Conclusion The average hospitalization days of the sample hospitals have a great potential to be shortened.Hospital A's cost control from 2018 to 2021 was not as expected,and the implementation of DRG has shown significant results in cost control.Hospital B's hospitalization cost structure is stable and tends to be optimized.
