尿液分析仪与红细胞镜检应用于尿液隐血检验中的效果比较

摘　　要：目的 比较尿液分析仪与红细胞镜检在尿液隐血检验中的应用效果。方法 对2022年6月—2023年6月于厦门市中医院进行尿液隐血检验的受检者中选择500例作为研究对象。所有研究对象均进行尿液分析仪检测、红细胞镜检,以尿液离心沉淀镜检结果作为“金标准”。比较尿液分析仪与红细胞镜检的检测结果及诊断效能。结果 尿液分析仪检验结果中,阳性166例,阴性334例,检出率为33.20%,红细胞镜检结果中,阳性167例,阴性333例,阳性检出率为33.40%;尿液分析仪Kappa值为0.810,一致性强度为最强,差异无统计学意义(P> 0.05);红细胞镜检Kappa值为0.743,一致性强度为高度,差异无统计学意义(P>0.05)。尿液分析仪的诊断效能:敏感度为87.80%,特异度为93.45%,准确度为91.60%;红细胞镜检的诊断效能:敏感度为83.54%,特异度为91.07%,准确度为88.60%。结论 尿液分析仪与红细胞镜检应用在尿液隐血检验中的诊断效能相近,尿液分析仪诊断效能略高于红细胞镜检,两者各有优势与不足。Objective To compare the application effect of urine analyzer and red blood cell microscopy in the detection of urinary occult blood.Methods From June 2022 to June 2023,a total of 500 subjects who underwent urine occult blood tests in Xiamen Traditional Chinese Medicine Hospital were selected as the study subjects.All subjects were tested by urine analyzer and red blood cell microscopy,and the results of urine centrifugation precipitation microscopy were taken as the gold standard.The results and diagnostic efficacy of urine analyzer and red blood cell microscopy were compared.Results In the urine analyzer test results,166 cases were positive and 334 cases were negative,with a detection rate of 33.20%.In the red blood cell microscopy test results,167 cases were positive and 333 cases were negative,with a positive detection rate of 33.40%.The Kappa value of the urine analyzer was 0.810,with the strongest consistency intensity and no statistically significant difference(P>0.05).The Kappa value of red blood cell microscopy was 0.743,and the consistency intensity was high,with no statistically significant difference(P>0.05).The diagnostic efficiency of the urine analyzer:sensitivity wss 87.80%,specificity was 93.45%,and accuracy was 91.60%.The diagnostic efficacy of red blood cell microscopy:sensitivity was 83.54%,specificity was 91.07%,and accuracy was 88.60%.Conclusion The diagnostic efficacy of urine analyzer and red blood cell microscopy in the detection of urinary occult blood is similar,the diagnostic efficacy of red blood cell microscopy is slightly higher than that of urine analyzer,both have advantages and disadvantages.
