摘 要:目的探讨高流量吸氧联合噻托溴铵粉吸入剂治疗慢性阻塞性肺疾病与阻塞性睡眠呼吸暂停(COPD-OSAHS)重叠综合征的效果。方法将90例COPD-OSAHS重叠综合征患者根据治疗方式不同分为对照组(45例,常规治疗)和观察组(45例,在常规治疗基础上采用高流量吸氧联合噻托溴铵粉吸入剂治疗)。比较两组的治疗效果。结果治疗后,观察组的第1秒用力呼气容积占预计值的百分比(FEV_(1)%)、一秒钟用力呼气容积占用力肺活量的比值(FEV_(1)/FVC)高于对照组,慢阻肺患者自我评估测试(CAT)评分、呼吸暂停低通气指数(AHI)、呼吸总阻抗(Zrs)、呼吸道总粘性阻力(R_(5))、周边阻力(R_(p))低于对照组(P<0.05)。治疗后,观察组的超氧化物歧化酶(SOD)、总抗氧化能力(TAC)水平高于对照组,丙二醛(MDA)水平低于对照组(P<0.05)。结论高流量吸氧联合噻托溴铵粉吸入剂治疗COPD-OSAHS重叠综合征的效果显著,有助于改善患者的呼吸紊乱情况、氧化应激指标,值得推广。Objective To investigate the effect of high flow oxygen inhalation combined with tiotropium bromide powder inhalation in the treatment of chronic obstructive pulmonary disease and obstructive sleep apnea-hypopnea syndrome(COPD-OSAHS)overlap syndrome.Methods According to different treatment methods,90 patients with COPD-OSAHS overlap syndrome were divided into control group(45 cases,routine treatment)and observation group(45 cases,treated with high flow oxygen inhalation combined with tiotropium bromide powder inhalation on the basis of routine treatment).The therapeutic effects of the two groups were compared.Results After treatment,the percentage of forced expiratory volume in first second to predicted value(FEV_(1)%)and the ratio of the forced expiratory volume in one second to forced vital capacity(FEV_(1)/FVC)in the observation group were higher than those in the control group,the COPD Assessment Test(CAT)score,apnea hypopnea index(AHI),total respiratory impedance(Zrs),total airway viscous resistance(R_(5))and peripheral airways(R_(p))were lower than those of the control group(P<0.05).After treatment,the levels of superoxide dismutase(SOD)and total antioxidant capacity(TAC)in the observation group were higher than those in the control group,and the level of malonaldehyde(MDA)was lower than that in the control group(P<0.05).Conclusion High flow oxygen inhalation combined with tiotropium bromide powder inhalation in the treatment of COPD-OSAHS overlap syndrome has significant effect,which is helpful to improve the respiratory disorder and oxidative stress indexes of patients,and is worthy of promotion.
