孕妇基本指标对延期妊娠引产效果的影响

摘　　要：目的分析孕妇年龄、身高、身体质量指数(BMI)、胎儿体重等与引产失败的关系。方法选取2017年8月至2021年8月西北妇女儿童医院产科收治的171例具有引产指征的延期妊娠孕妇病历资料进行回顾性分析,根据引产是否成功将其分为引产失败组(87例)和引产成功组(84例)。比较两组的年龄、身高、BMI、胎儿体重等指标。结果引产后,引产成功组的Bishop评分高于引产失败组(P<0.05)。引产失败组的缩宫素、米索前列醇使用次数、引产天数多于引产成功组(P<0.05);引产失败组的宫颈扩张球囊使用次数多于引产成功组,但差异无统计学意义(P>0.05)。引产失败组的年龄、孕前体重、分娩前体重、孕前BMI、分娩前BMI及婴儿体重大于引产成功组,身高低于引产成功组,流产次数多于引产成功组(P<0.05)。结论孕妇基本指标显著影响引产成功率,年龄越大、体重越高、身高越低、BMI越高、流产次数越多、胎儿体重越大的孕妇引产成功率越低,避免非计划妊娠、加强体重和饮食管理、控制胎儿体重对顺利分娩具有重要意义。Objective To analyze the relationship between pregnant women age,height,body mass index(BMI),fetal weight and induced labor failure.Methods From August 2017 to August 2021,the medical records of 171 delayed pregnancy pregnant women with indications of induced abortion admitted in the obstetrics department of Northwest Women's and Children's Hospital were retrospectively analyzed.According to whiether incluced abortion was successful,the pregnant women were divided into induced labor failure group(87 cases)and induced labor success group(84 cases).The age,height,BMI,fetal weight and other indicators of the two groups were compared.Results After induced labor,the Bishop score of the induced labor success group was higher than that of the induced labor failure group(P<0.05).The number of oxytocin and misoprostol used and the number of induced labor days in the induced labor failure group were more than those in the induced labor success group(P<0.05);the number of cervical dilatation balloons used in the induced labor failure group was more than that in the induced labor success group,but the differences were not statistically significant(P>0.05).The age,pre-pregnancy weight,pre-delivery weight,pre-pregnancy BMI,pre-delivery BMI and fetal weight of the induced labor failure group were greater than those of the induced labor success group,the height was shorter than that of the induced labor success group,and the number of abortions was more than that of the induced labor success group(P<0.05).Conclusion The basic indicators of pregnant women significantly affect the success rate of induced labor.The older the age,the higher the weight,the shorter the height,the higher the BMI,the more the number of abortions,and the greater the fetal weight,the lower the success rate of induced labor.Avoiding unplanned pregnancy,strengthening weight and diet management,and controlling fetal weight are of great significance for smooth delivery.
