摘 要:心血管自主神经病变(CAN)是2型糖尿病最常见但最易被忽略的并发症之一,其起病隐匿,随病程进展可引起心肌缺血、心律失常甚至心源性猝死等,预后不佳。目前针对CAN治疗还只局限于延缓进展及控制症状。同型半胱氨酸(Hcy)是蛋氨酸代谢的中间产物,研究表明升高的Hcy可通过氧化应激、炎症等多种途径损伤神经系统。本文主要论述Hcy在CAN发生、发展中的可能作用机制,旨在为将来通过降低血浆Hcy来改善CAN提供理论依据。Cardiovascular autonomic neuropathy(CAN)is one of the most common but easily overlooked complications of type 2 diabetes mellitus.It has an insidious onset and can cause myocardial ischemia,arrhythmia and even sudden cardiac death with the progression of the disease,and the prognosis is poor.At present,the treatment for CAN is only limited to delaying progress and controlling symptoms.Homocysteine(Hcy)is an intermediate product of methionine metabolism.Studies have shown that elevated Hcy can damage the nervous system through oxidative stress,inflammation and other pathways.This article mainly discusses the possible mechanism of Hcy in the occurrence and development of CAN,so as to provide a theoretical basis for improving CAN by reducing plasma Hcy in the future.
