摘  要:目的:探讨卵巢储备功能下降(diminished ovarian reserve,DOR)患者微刺激体外受精/卵胞浆内单精子注射(in vitro fertilization/intracytoplasmic sperm injection,IVF/ICSI)周期中BMI与优质胚胎形成的关系。方法:回顾性分析2019年1月―2021年7月因DOR首次采用微刺激方案的88个周期,记录患者的年龄、BMI、不孕年限等基线资料。根据优质胚胎形成将患者分为两组(有优胚组和无优胚组),采用独立样本t检验、Mann-Whiteny U检验、χ^(2)检验或Fisher精确检验分析两组相关指标的差异性,多因素Logistic回归分析基线资料与优质胚胎形成之间的关系,构建ROC曲线,寻找预测DOR患者微刺激IVF/ICSI周期中优质胚胎形成指标的界值。结果:有优胚组患者的BMI明显低于无优胚组[(21.32±2.61)kg/m^(2)vs(22.89±3.12)kg/m^(2),P<0.05]。多因素Logistic回归分析示BMI是DOR患者微刺激周期中优质胚胎形成的独立影响因子(OR=0.817,95%CI:0.693~0.964,P=0.017);ROC曲线示BMI预测DOR患者优质胚胎形成的界值为21.45 kg/m^(2)。结论:BMI是DOR患者微刺激IVF/ICSI周期中优质胚胎形成的独立预测因素。控制BMI对DOR患者的胚胎发育结局具有重要价值。Objective:To explore the relationship between BMI and the high quality embryo formationin patients with diminished ovarian reserve(DOR)in mild stimulation in vitro fertilization/intracytoplasmic sperm injection(IVF/ICSI)cycles.Methods:From January 2019 to July 2021,88 mild stimulation IVF/ICSI cycles were recruited.Age,BMI,duration of infertility and other baseline data were recorded.Patients were divided into two groups based on the high quality embryo formation(with high-quality embryos group vs without high-quality embryos group).Independent sample t-test,Mann-Whitey U test,χ^(2)test or Fisher exact test were used to analyze the differences of relevant indicators between the two groups.Multivariate Logistic regression analysis was used to explore the relationship between relevant indicators and high quality embryo formation.Then ROC analysis was performed to find the cut off number of the relevant indicators for predicting the outcome of high quality embryo formation of DOR patients in mild stimulation IVF/ICSI.Results:The BMI of the group with high-quality embryos was significantly lower than that of the group without high-quality embryos[(21.32±2.61)kg/m^(2)vs(22.89±3.12)kg/m^(2),P<0.05].Moreover,multivariate Logistic regression analysis showed that BMI was the independent factor affecting the formation of high-quality embryos during mild stimulation cycles in patients with DOR embryo(OR=0.817,95%CI:0.693-0.964,P=0.017).Furthermore,ROC curve showed that the threshold value for BMI in predicting the outcome of high quality embryo formation was 21.45 kg/m^(2).Conclusion:BMI is an independent predictor of high quality embryo formation in DOR patients in mild stimulation IVF/ICSI.Controling of BMI is of great value to the embryonic developmental outcome of DOR patients.
