基层医院绝经妇女宫腔镜术前不同用药方案对宫颈预处理及术后安全性的研究

摘　　要：目的分析基层医院绝经妇女宫腔镜术前不同用药方案对宫颈预处理及术后安全性。方法选取60例行宫腔镜检查患者,按照处理方法不同分为米索前列醇组、间苯三酚组、联合组,各20例。比较三组临床效果。结果联合组的宫颈软化有效率95%明显高于米索前列醇组80%、间苯三酚组70%(P<0.05)。相比米索前列醇组、间苯三酚组,联合组宫颈内口宽度较大,扩宫时间、手术时间较短,膨宫液使用量、术中出血量较少(P<0.05)。联合组不良反应发生率低于米索前列醇组(P<0.05)。处理后联合组收缩压(SBP)、舒张压(DBP)、心率(HR)指标变化小于米索前列醇组、间苯三酚组(P<0.05)。结论宫腔镜检查前应用间苯三酚联合米索前列醇,可提升宫颈软化程度,且安全性较高,对血流动力学的影响小,是比较理想的预处理方案。Objective To analyze the safety of different medication plans before hysteroscopy for cervical pretreatment and postoperative treatment of menopausal women in primary hospitals.Methods 60 patients undergoing hysteroscopy were selected and divided into misoprostol group,phloroglucinol group and combined group(20 cases each)according to different treatment methods.The clinical effects of the three groups were compared.Results The effective rate of cervical softening in combination group(95%)was significantly higher than that in misoprostol group(80%)and phloroglucinol group(70%)(P<o.05).Compared with misoprostol group and phloroglucinol group,the combined group had larger cervical opening width,shorter uterine dilation time and operation time,less uterine swelling fluid consumption and less intraoperative blood loss(P<0.05).The incidence of adverse reactions in combination group was lower than that in misoprostol group(P<0.05).After treatment,the changes of systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR)in combination group were lower than those in misoprostol and phloroglucinol groups(P<o.05).Conclusion The application of phlorogliol combined with misoprostol before hysteroscopy can improve the degree of cervical softening,and has high safety and lttle influence on hemodynamics,so it is a relatively ideal pretreatment program.
