经阴道彩色多普勒超声鉴别宫外孕破裂与卵巢黄体破裂的临床有效性探讨

摘　　要：目的研究经阴道彩色多普勒超声鉴别宫外孕破裂与卵巢黄体破裂的临床有效性。方法选取55例卵巢黄体破裂患者作为对照组,50例宫外孕破裂患者作为观察组,均经手术或病理检查证实。两组均进行经阴道彩色多普勒超声检查。比较两组超声特征以及血流相关信息。结果卵巢黄体破裂的诊断准确率为90.91%,宫外孕破裂的诊断准确率为96.00%,比较差异无统计学意义(P>0.05)。对照组血流量以(半)环状为主,观察组以点状血流居多,对比差异有统计学意义(P<0.05)。在血流频谱特征方面,对照组、观察组分别以低阻力型、高阻力型居多,且对比差异有统计学意义(P<0.05)。观察组的阻力指数与搏动指数均比对照组高,而收缩期峰值流速则比对照组慢(P<0.05)。结论经阴道彩色多普勒超声检查结合超声特征、血流相关信息能够准确鉴别诊断宫外孕破裂、卵巢黄体破裂,便于后续尽早开展对症治疗。Objective To study the clinical effectiveness of transvaginal color doppler ultrasound in differentiating rupture of ectopic pregnancy from rupture of luteal corpus of ovary.Methods 55 patients with ruptured corpus luteum of ovary were selected as control group,and 50 patients with ruptured ectopic pregnancy were selected as observation group,all of which were confirmed by operation or pathological examination.Transvaginal color doppler ultrasound was performed in both groups.The ultrasound characteristics and blood flow related information were compared between the two groups.Results The diagnostic accuracy of ruptured corpus luteum of ovary was 90.91%and that of ruptured ectopic pregnancy was 96.00%,and the difference was not statistically significant(P>0.05).The blood flow in the control group was mainly(semi-)annular,and the point blood flow in the observation group was mostly,and the difference was statistically significant(P<0.05).In terms of blood flow spectrum characteristics,low resistance type and high resistance type were the majority in control group and observation group,respectively,and the difference was statistically significant(P<0.05).The resistance index and pulse index of the observation group were higher than those of the control group,and the peak systolic flow rate was slower than that of the control group(P<o.05).Conclusion Transvaginal color doppler ultrasonography combined with ultrasound characteristics and relevant information of blood flow can accurately differentiate the diagnosis of ectopic pregnancy rupture and luteal ovary rupture,which is convenient to carry out symptomatic treatment as soon as possible.
