摘 要:目的:观察大黄素载药骨水泥对感染性创面修复的作用及其作用机制。方法:60只大鼠平均分为空白组、模型组、大黄素骨水泥组(PMMA+E)和万古霉素骨水泥组(PMMA+V),除空白组外均构建金黄色葡萄球菌感染的大鼠背部全层皮肤切除模型;于干预后第1、7、14天观察大鼠创面面积愈合情况;于干预后第7、14天检测各组创面菌落数,HE染色法观察创面组织病理学变化,ELISA法检测大鼠血清中白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)含量,RT-qPCR法检测创面中IL-6、TNF-αmRNA表达水平;Western-Blot检测创面中TLR4/NF-κB信号通路蛋白表达水平。结果:干预后第7、14天,与模型组、PMMA+V比较,PMMA+E组大鼠创面面积明显缩小(P<0.05)。与模型组比较,PMMA+E组菌落数明显减少(P<0.05);PMMA+E组与PMMA+V组比较无明显差异。与模型组、PMMA+V组比较,PMMA+E组HE染色可见炎症细胞浸润减少,大量血管新生。与模型组、PMMA+V组比较,PMMA+E组大鼠血清IL-6、TNF-α含量明显降低(P<0.05)。与模型组比较,PMMA+E组大鼠创面IL-6 mRNA含量明显降低(P<0.05);PMMA+E组与PMMA+V组比较无明显差异。与模型组、PMMA+V组比较,PMMA+E组大鼠创面TNF-αmRNA含量明显降低(P<0.05)。与模型组比较,PMMA+E组大鼠创面组织中Toll样受体4(TLR4)、NF-κB蛋白表达降低(P<0.05);PMMA+E组与PMMA+V组比较无明显差异。结论:外用大黄素能有效抑制金黄色葡萄球菌生长,抑制感染性创面大鼠血清炎症因子水平以及创面炎症因子水平,从而促进创面愈合,其作用机制可能与TLR4/NF-κB信号通路有关。Objective To observe the effect of Emodin-loaded bone cement on the repair of infectious wound and to study the mechanism.Methods Sixty rats were divided into normal group,model group,Emodin-loaded bone cement group(PMMA+E)and vancomycin bone cement group(PMMA+V),except for the blank group,a model of total skin excision on the back of rats infected with Staphylococcus aureus was constructed.The number of colonies in each group was measured on days 1,7 and 14 after the intervention,and the histopathological changes of the wounds were observed by HE staining,IL-6 and TNF-αin the serum of rats were detected by ELISA,and IL-6 and TNF-αin the wounds were detected by RT-qPCR.Western-Blot detection of TLR4/NF-κB signaling pathway protein expression levels in wounds.Results On days 7 and 14 after the intervention,the wound area of rats in the PMMA+E group was significantly reduced compared with the model group and PMMA+V group(P<0.05).Compared with the model group,the number of colonies in the PMMA+E group was significantly reduced(P<0.05);compared with the PMMA+V group,there was no significant difference.Compared with the model group and PMMA+V group,HE staining in the PMMA+E group showed a decrease in inflammatory cell infiltration and a large number of new blood vessels.Compared with the model group and PMMA+V group,the serum IL-6 level was significantly lower and TNF-αlevel was significantly lower(P<0.05)in the PMMA+E group.Compared with the model group,the wound IL-6 mRNA content of rats in the PMMA+E group was significantly lower(P<0.05);compared with the PMMA+V group,there was no significant difference.Compared with the model group and PMMA+V group,the wound TNF-αmRNA content of rats in the PMMA+E group was significantly lower(P<0.05).Compared with the model group,the expression of TLR4 and NF-κB protein in the traumatic tissue of rats in the PMMA+E group was reduced(P<0.05);compared with the PMMA+V group,there was no significant difference.Conclusion Topical emodin can effectively inhibit the growth of Stap
