预防性手指操降低ICU住院患者冠状动脉造影术后并发症的效果

摘　　要：目的分析预防性手指操降低ICU住院患者冠状动脉造影术后并发症的效果。方法调取2020年8月至2021年8月诊治的采取冠状动脉造影检查的ICU住院患者64例,作为研究对象。按照随机数字表法方式,分为2个护理单元。对照组32例,予以系统护理,包括健康教育、心理辅导、日常护理、生命体征监测和针对并发症的特殊护理。观察组32例,在系统护理基础上,予以预防性手指操训练,术后30 min开始,每小时重复1组动作。对观察指标进行分析,包括术后手掌周径差、术后视觉模拟量表评分(VAS)和术后并发症。结果对照组术后1 h、2 h、3 h、4 h、5 h、6 h的手掌周径差,均高于观察组,差异有统计学意义(P<0.05)。对照组术后1 h、2 h、4 h、6 h的术后疼痛程度均高于观察组,差异有统计意义学(P<0.05)。对照组术后总并发症率18.75%,高于观察组3.12%,差异有统计学意义(P<0.05)。结论ICU住院患者冠状动脉造影术后预防性手指操训练可以减轻术后疼痛及肢体肿胀,减少术后并发症的出现,干预效果显著,值得大力推广。Objective To analyze the effect of preventive finger exercises on reducing complications in intensive care unit(ICU)inpatients after coronary angiography.Methods In this study,64 inpatients in ICU examined by coronary angiography from August 2020 to August 2021 were recruited.They were randomly divided into control group and observation group,with 32 cases per group.Patients in the control group were managed by systematic nursing,including health education,psychological counseling,daily nursing,vital sign monitoring and special nursing for complications.Patients in the observation group were given preventive finger exercise training at 30 min postoperatively(1 group of exercise per hour)and systematic nursing.The observation indexes,including the postoperative difference of palm circumference,the Visual Analogue Scale(VAS)score and postoperative complications,were analyzed.Results The difference of palm circumference in the control group at 1h,2h,3h,4h,5h and 6h postoperatively was significantly higher than that of the observation group(P<0.05).The degree of postoperative pain in the control group at 1h,2h,4h and 6h postoperatively was significantly higher than that of the observation group(P<0.05).The total postoperative complication rate in the control group was significantly higher than that of the observation group(18.75%vs 3.12%,P<0.05).Conclusion Preventive finger exercise training for ICU inpatients after coronary angiography can reduce postoperative pain and limb swelling,and postoperative complications.The intervention effect is significant,and it is worth promoting.
