摘　　要：目的研究以家庭为中心的探视模式对机械通气患者康复效果的影响。方法选取2016至2018年重症监护室(ICU)接受机械通气的250例患者为此次研究对象,随机数字表法分为对照组和研究组,每组125例。对照组采用常规探视模式,研究组采用以家庭为中心的探视模式,比较2组患者的ICU综合征发生率、康复情况(机械通气时间、入住ICU时间、一次脱机成功率)、患者家属疾病不确定感、满意度。结果干预后,研究组患者的ICU综合征发生率低于对照组(P<0.05);研究组的机械通气时间、入住ICU时间均短于对照组,一次脱机成功率高于对照组(P<0.05);研究组患者家属的不确定、复杂、信息缺乏、不可预测性评分均低于对照组,患者家属保证病情、信息获取、被接纳、获得支持、舒适度评分均高于对照组(P<0.05)。结论在机械通气患者中实施以家庭为中心的探视模式干预,能降低患者ICU综合征发生率,加快患者康复进程,减轻患者家属的疾病不确定感,提高患者家属满意度。Objective To study the effect of family-centered visitation mode on the rehabilitation of patients managed by mechanical ventilation.Methods A total of 250 patients managed by mechanical ventilation in intensive care unit(ICU)of our hospital from 2016 to 2018 were recruited.They were randomly divided into control group and study group,with 125 cases in each group.Routine visitation mode and family-centered visitation mode were applied to patients of control group and study group,respectively.The incidence of ICU syndrome,recovery status(mechanical ventilation time,length of stay in ICU,success rate of ventilator weaning),and disease uncertainty and satisfaction of family members were compared between the two groups.Results After intervention,the incidence of ICU syndrome in the study group was significantly lower than that of control group(P<0.05).The mechanical ventilation time and length of stay in ICU were significantly shorter in the study group than those of the control group,and the success rate of ventilator weaning was significantly higher(P<0.05).The scores of disease uncertainty,complexity,lack of information and unpredictability of family members in the study group were significantly lower than those of the control group(P<0.05).The scores of disease condition guarantee,information access,acceptance,support and comfort level of family members in the study group were significantly higher than those of the control group(P<0.05).Conclusion Family-centered visitation intervention in mechanical ventilation patients can lower the incidence of ICU syndrome,accelerate the process of rehabilitation,reduce the uncertainty of family members and improve their satisfaction.
