孕早期血清C反应蛋白、血清铁蛋白检测与妊娠糖尿病发生风险的相关性研究

摘　　要：目的:探讨孕早期血清C反应蛋白(CRP)、血清铁蛋白在妊娠糖尿病(GDM)期间的预测价值,为临床防治GDM提供理论依据。方法:收集对照组健康孕妇及GDM孕妇临床相关资料及孕10周时的血清样本,检测血清CRP、血清铁蛋白、空腹血糖、糖化血红蛋白水平。结果:GDM孕妇血清CRP水平、血清铁蛋白均高于对照组健康孕妇(P<0.01)。血清CRP与空腹血糖、糖化血红蛋白、血清铁蛋白与空腹血糖、糖化血红蛋白均呈正相关关系(P<0.01)。ROC曲线分析结果显示,血清CRP、血清铁蛋白、2组联合对GDM的早期预测均具有一定的临床价值(P<0.05)。结论:孕早期血清CRP、血清铁蛋白水平变化与GDM密切相关,血清CRP、血清铁蛋白对预测GDM发病有一定的价值,而且两者联合预测的效能更高。Objective:To explore the predictive value of serum levels of C-reactive protein(CRP)and ferritin in early pregnancy on gestational diabetes mellitus(GDM),and provide a theoretical basis for clinical prevention and treatment of GDM.Methods:The clinical data of healthy pregnant women and GDM pregnant women,and serum samples at 10 weeks of gestation were collected.The serum levels of CRP and ferritin,fasting blood glucose and glycosylated hemoglobin were detected.Results:The serum levels of CRP and ferritin in pregnant women with GDM were significantly higher than those in healthy pregnant women(P<0.01).Both the serum levels of CRP and ferritin were positively correlated with the fasting blood glucose and glycosylated hemoglobin of pregnant women with GDM(P<0.01).The results of ROC curve analysis showed that the combination detection of serum CRP and ferritin had certain clinical value in the early prediction of GDM(P<0.05).Conclusions:The changes of serum levels of CRP and ferritin in early pregnancy are closely related to the GDM.The serum CRP and ferritin have a certain value in predicting the onset of GDM,and the combination of the two is more effective in predicting the onset of GDM.
