髋关节置换病人居家跌倒敏感指标构建及干预研究

摘　　要：目的:探讨以敏感指标构建为基础的护理干预在髋关节置换病人居家防跌倒中的应用。方法:选取2019年收治的初次人工全髋关节置换病人34例作为对照组,实施常规护理;2020年收治的初次人工全髋关节置换病人40例作为观察组,采用基于髋关节居家跌倒敏感指标构建的护理干预。比较2组病人干预后居家环境改善率、跌倒效能评分和髋关节Harris评分。结果:出院后1个月,除衣服和鞋子评估与对照组差异无统计学意义(P>0.05)外,观察组其余各项居家环境改善落实率均高于对照组(P<0.05~P<0.01)。2组出院时跌倒效能、髋关节Harris评分差异均无统计学意义(P>0.05);术后1、3个月,2组跌倒效能、髋关节Harris评分均随时间增加而明显增加(P<0.01),且观察组跌倒效能、髋关节Harris评分均高于对照组(P<0.05~P<0.01)。结论:基于髋关节居家跌倒敏感指标构建的护理干预,能改善病人居家安全环境,提高病人跌倒效能,促进髋关节功能恢复。Objective:To study the application of nursing intervention based on the construction of sensitive indicators in preventing falling of hip replacement patients.Methods:A total of 34 patients with the first total hip replacement admitted in 2019 were selected as the control group,and routine nursing was carried out;40 patients with the first total hip replacement in 2020 were selected as the observation group,and nursing intervention constructed by sensitive indicators based on falls at home of hip joint was adopted.The improvement rate of home environment,fall efficacy score and Harris hip score were compared between the two groups after intervention.Results:One month after discharge,except that there was no significant difference in the evaluation of clothes and shoes(P>0.05),the implementation rate of other home environment improvements in the observation group was higher than that in the control group(P<0.05 to P<0.01).There was no significant difference in the fall efficacy and Harris score of hip joint between the two groups at discharge(P>0.05).At 1 and 3 months after operation,the fall efficacy and Harris score of hip joint in both groups increased significantly with time(P<0.01),and the fall efficacy and Harris score of hip joint in the observation group were higher than those in the control group(P<0.05 to P<0.01).Conclusions:Nursing intervention constructed by sensitive indicators based on falls at home of hip joint can improve the safe home environment of patients,improve the fall efficiency of patients,and promote the recovery of hip function.
