基于药症相应探讨历代本草辨治痒症思路

摘　　要：目的基于药症相应,探讨历代本草文献中治疗痒症的思路。方法检索《中华医典》数据库中本草类的全部文献数据,收集整理治疗痒症的本草信息,对筛选纳入的本草(中药)进行性味、归经统计分析,以痒症辨治病机要素进行分类统计,提取各类本草(中药)的辨治内容。结果在76部历代本草专著中,筛选出119味治疗痒症的本草(中药)。治疗痒症的本草(中药)药性以温性、寒性、平性为主,药味以苦味、辛味、甘味为主,归经以肝经、肺经为主,病机要素为风邪袭扰、湿邪闭阻、火热毒损、气血郁闭、正虚失荣。结论痒症辨治常用祛风散邪药、祛湿化浊药、清热泻火药、理气活血药、补虚扶正药,且药物之间多互相配伍使用;邪扰、郁闭、正虚为痒症辨治主要病机特点,符合邪气袭扰作祟、气血郁闭不通、正气亏虚不足的机体功能紊乱状态。Objective Based on the correspondence between drugs and symptoms,this paper discusses the thinking of treatment of pruritus in the literature of materia medica in the past dynasties.Methods Retrieve all the literature data of materia medica in the database of Chinese Medical Dictionary,and collect and sort out the information of materia medica for treatment of pruritus,the nature,taste and channel tropism of the selected herbal medicines were statistically analyzed,and the pathogenesis elements of pruritus were classified and counted,the contents of syndrome differentiation and treatment of various herbal medicines were extracted.Results In 76 monographs on materia medica in the past dynasties,119 herbal medicines for the treatment of pruritus were selected.Herbal medicines for treating pruritus is mainly warm,cold and mild in nature,bitter,pungent and sweet in taste,and liver and lung meridians in channel tropism.In the herbal classification of pruritus pathogenesis elements,it can be divided into five categories of pathogenesis elements:wind invasion,dampness obstruction,fire-heat toxin damage,qi and blood stagnation,and deficiency of vital qi.Conclusions Pruritus is mainly treated with dispelling pathogenic wind and evil,removing dampness,clearing heat and purging fire,regulating qi and activating blood circulation,tonifying deficiency and strengthening body resistance,which are used in combination with each other.Disturbance of pathogenic factors,stagnation and deficiency of vital qi are the main characteristics of syndrome differentiation and treatment of pruritus,which is in line with the physiological dysfunction state of attack and disturbance of pathogenic qi,stagnation of qi and blood,and the deficiency of vital qi.
