摘　　要：强直性脊柱炎的致病因素不外乎“虚、瘀、郁”3个核心病机,具体表现为先天肾虚督空、后天脾胃失养、瘀血互结、肝气郁结。临证巧用药对治以补先天肾气,调后天脾胃,活血通经,畅达情志。狗脊-杜仲相须使用,协同发挥温补固摄、补肾强督之效,可用于治疗肝肾虚损,腰膝酸软,下肢无力等症状,标本兼治肾虚腰痛;当归-熟地黄配伍,既能益精填髓、调经止痛,又能增强自身正气;乳香-没药合用,活血、止痛、消肿、抗炎;全蝎-蜈蚣合用,补气、化瘀、消痰、止痛;薏苡仁-茯苓配伍使用,滋养后天之本,脾胃主肌肉四肢,保护筋骨,脾胃健旺,筋骨得以滋养,故对缓解关节僵硬、疼痛有明显的治疗作用;柴胡-黄芩从肝论治,养肝疏肝,使肝血得以濡养,肝气得以条达,缓解焦虑抑郁。The pathogenic factors of ankylosing spondylitis are no more than the three core pathogenesis of"deficiency,blood stasis,and Qi stagnation",which are manifested as congenital Kidney deficiency and Du meridian deficiency,Spleen and Stomach deficiency,mutual accumulation of blood stasis,and Liver Qi stagnation.Skilled medication is used to supplement congenital Kidney Qi,regulate the acquired Spleen and Stomach,promote blood circulation and meridian circulation,and ease emotions.The combination of Gouji(rhizoma cibotii)and Duzhong(eucommia ulmoides)can synergistically exert the effects of warming,tonifying,firming,tonifying the Kidney and strengthening Du meridian.It can be used to treat symptoms such as Liver and Kidney deficiency,waist and knee soreness,and lower limb weakness.It can also treat Kidney deficiency and lower back pain.The combination of Danggui(angelica sinensis)and Shoudihuang(rehmannia glutinosa)can not only benefit the Essence and fill the marrow,regulate menstruation and relieve pain but also enhance man's vital energy;The combination of Ruxiang(frankincense)and Moyao(myrrh)can promote blood circulation,relieve pain,reduce swelling,and anti-inflammatory effects;Quanxie(scorpion)and Wugong(centipede)combination function in tonifying Qi,resolving blood stasis,reducing Phlegm,and relieving pain.The combination of Yiyiren(coix seed)and Fuling(poria cocos)nourishes the acquired Essence,with Spleen and Stomach controlling the muscles and limbs,protecting the muscles and bones.The Spleen and Stomach are healthy,nourishing the muscles and bones,thus having a significant therapeutic effect on relieving joint stiffness and pain;Chaihu(radix bupleuri)and Huangqin(scutellaria baicalensis)works from perspective of Liver,nourishing and soothing the Liver,nourishing Liver blood,regulating Liver Qi,and alleviating anxiety and Qi stagnation.
