补中益气五苓汤加减联合雷火灸治疗高血压性心力衰竭气虚水停证疗效研究

摘　　要：目的:探讨补中益气五苓汤加减联合雷火灸治疗高血压性心力衰竭气虚水停证的疗效。方法:纳入高血压性心力衰竭气虚水停证患者136例,随机分为对照组和观察组,每组68例。对照组采用西医常规治疗,观察组在对照组基础上联合补中益气五苓汤加减及雷火灸治疗,治疗8周。比较两组临床疗效、心功能、炎症因子指标等。结果:治疗后观察组血清转化生长因子-β1(TGF-β1)、血栓素B2(TXB2)、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平低于对照组,差异有统计学意义(均P<0.05)。治疗后观察组血清N-末端脑钠肽前体(NT-proBNP)、醛固酮(ALD)、内皮素-1(ET-1)水平低于对照组,差异有统计学意义(均P<0.05)。治疗后观察组左室射血分数(LVEF)大于对照组,左心室后壁厚度(LVPW)、左室舒张末期内径(LVEDD)低于对照组(均P<0.05)。观察组总有效率94.12%,高于对照组的82.35%(P<0.05)。结论:补中益气五苓汤加减及雷火灸治疗高血压性心力衰竭气虚水停证患者有助于减轻炎症反应,改善心功能,延缓心室重构,提高临床疗效。Objective:To investigate the curative effect of modified Buzhong Yiqi Wuling decoction combined with thunder fire moxibustion on qi-deficiency and water-stopping syndrome of hypertensive heart failure.Methods:136 patients with hypertensive heart failure syndrome of qi-deficiency and water-stopping were randomly divided into control group and observation group,with 68 cases in each group.The control group was treated with conventional western medicine,and the observation group was treated with modified Buzhong Yiqi Wuling decoction and thunder fire moxibustion for 8 weeks.The clinical efficacy,heart function,inflammatory factor index were compared between the two groups.Results:After treatment,the levels of serum transforming growth factor-β1(TGF-β1),thromboxane B2(TXB2),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in observation group were lower than those in control group,and the differences were statistically significant(all P<0.05).After treatment,the serum levels of N-terminal brain natriuretic peptide precursor(NT-proBNP),aldosterone(ALD)and endothelin-1(ET-1)in observation group were lower than those in control group,and the differences were statistically significant(all P<0.05).After treatment,the left ventricular ejection fraction(LVEF)in the observation group was higher than that in the control group,the left ventricular posterior wall thickness(LVPW)and left ventricular end-diastolic diameter(LVEDD)were lower than that in the control group(all P<0.05).The total effective rate of observation group was 94.12%,which was higher than that of control group(82.35%)(P<0.05).Conclusion:Modified Buzhong Yiqi Wuling decoction combined with thunder fire moxibustion in the treatment of hypertensive heart failure patients with qi-deficiency and water-stopping syndrome can help reduce inflammation,improve heart function,delay ventricular remodeling and improve clinical efficacy.
