复元活血汤加味对胸腰椎骨折内固定患者术后炎症因子水平及腰椎功能的影响

摘　　要：目的探讨复元活血汤加味对胸腰椎骨折内固定患者术后炎症因子水平及腰椎功能的影响。方法前瞻性研究,纳入2020年1月至2023年1月泰和县中医院行胸腰椎骨折内固定术患者88例作为研究对象,以随机数字法分为对照组和观察组,各44例。对照组给予常规治疗,观察组于对照组基础上加用复元活血汤加味治疗,两组均持续治疗6个疗程,比较两组患者临床疗效,炎症因子[白细胞介素-8(IL-8)、超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)]以及腰椎功能[伤椎顶部高度、伤椎中部高度]。结果观察组总有效率较对照组高,差异有统计学意义(P<0.05)。治疗后,两组患者血清TNF-α、hs-CRP及IL-8水平均较干预前降低,且观察组各指标变化更显著(P<0.05);伤椎顶部高度、伤椎中部高度较治疗前升高,且观察组各指标水平变化更突出(P<0.05)。结论复元活血汤加味对胸腰椎骨折内固定患者疗效显著,可有效降低患者炎症因子水平,促进腰椎功能恢复。Objective To explore the effect of modified fuyuan huoxue decoction on postoperative inflammatory factor levels and lumbar function in patients with thoracolumbar spine fractures and internal fixation.Methods A prospective study was conducted,including 88 patients who underwent internal fixation surgery for thoracolumbar fractures in Taihe County Traditional Chinese Medicine Hospital from January 2020 to January 2023.They were randomly divided into a control group and an observation group,with 44 cases in each group.The control group received routine treatment,while the observation group received modified fuyuan huoxue decoction treatment on basis of the control group,both groups continued to receive treatment for 6 courses.The clinical efficacy,inflammatory factors[interleukin-8(IL-8),hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)]and lumbar spine function[top height of injured vertebrae,height of injured vertebrae in the middle]of the two groups were compared.Results The total effective rate of the observation group was higher than that of the control group after 6 courses of treatment,and the difference was statistically significant(P<0.05).6 courses of treatment,two groups of patients with serum TNF-α.The average levels of hs-CRP and IL-8 water decreased compared to before intervention,and the changes in various indicators in the observation group were more significant(P<0.05);the top height of the injured vertebrae and the height of the middle part of the injured vertebrae increased compared to before treatment,and the changes in the levels of various indicators in the observation group were more prominent(P<0.05).Conclusion The modified fuyuan huoxue decoction has a significant therapeutic effect on patients with thoracolumbar fractures and internal fixation,which can effectively reduce the level of inflammatory factors in patients and promote the recovery of lumbar function.
