采用PIM标准评价糖尿病合并高血压、冠心病老年患者用药安全的临床研究

摘　　要：目的研究联用潜在不适当用药(PIM)标准在识别和减少老年共病患者药物相关问题、优化药物治疗方案中的价值。方法选取萍乡市人民医院2019年1月1日至2020年12月31日诊断为2型糖尿病、原发性高血压、冠心病的老年共病患者70例,随机数字表法分为中国标准组和联合评价组,每组35例。中国标准组运用PIM中国标准筛选,联合评价组运用多个PIM标准(PIM中国标准+Beers标准+STOPP标准)联合评价。结果联合评价组的PIM检出率、PIM检出数量和用药依从性评分均显著高于中国标准组(P<0.05),次均药品费用两组差异无统计学意义(P>0.05)。结论运用PIM联合评价标准可发现和避免更多实际发生或潜在的药物相关问题,改善多重用药,提高患者用药依从性,减少社会医疗资源的浪费。Objective To study the potential inappropriate medication(PIM)standards in identifyind and reducing drug related issues in edly comorbidities and optimizing drug treatment plans.Methods 70 elderly patients with type 2 diabetes,hypertension and coronary heart disease admitted to Pingxiang People's Hospital from January 1st,2019 to December 31st,2020 were divided into control group and joint evaluation group by random number table method,with 35 patients in each group.The control group applied the PIM using the Chinese standards,and the joint evaluation group used the PIM joint evaluation criteria.Results The incidence of PIM,the number of PIM and medication adherence score in the joint evaluation group were higher than those in the control group(P<0.05),and the average drug expenses has no significant difference between the two groups(P>0.05).Conclution Using PIM criterions in elderly patients with comorbidity can significantly improve polypharmacy of patients,reduce drug-induced damage,improve patient compliance,and reduce the waste of medical resources.
