访视干预对老年糖尿病患者安全性与有效性的Meta分析

摘　　要：目的探究访视干预对老年糖尿病患者的影响。方法检索中国知网、维普、万方、PubMed、Cochrane Library、Embase等数据库中公开发表的与老年糖尿病患者访视干预相关的随机对照研究,检索时限为各数据库建库至2022年5月20日。采用RevMan 5.3软件进行Meta分析。结果共纳入18篇文献,Meta分析结果显示,干预后老年患者糖化血红蛋白(glycosylated hemoglobin,HbA1c)[SMD=-0.89,95%CI(-1.35,-0.44),P=0.0001]、餐后2 h血糖(2-hour postprandial blood glucose,2 h PG)[SMD=-0.90,95%CI(-1.27,-0.53),P<0.00001]、自我效能评分量表(Diabetes Self-efficacy Scale,DSES)评分[SMD=3.19,95%CI(2.52,3.86),P<0.00001],差异有统计学意义。结论访视干预能够改善老年糖尿病患者的糖化血红蛋白及2 h餐后血糖水平、自我效能水平,有助于提高患者家庭功能水平、生存及睡眠质量。Objective To investigate the effectiveness and safety of visitation interventions in elderly patients with diabetes mellitus.Methods We searched the randomized controlled studies related to visitation interventions in elderly patients with diabetes published in CNKI,VIP Database(VIP),Wanfang,PubMed,the Cochrane Library,EMbase and other databases.Databases were retrieved from the establishment of the library to May 20,2022.Review Manager 5.3 software was used for statistical analysis.Results A total of 18 literature entries were included,and the meta-analysis showed that after the intervention,elderly patients with hemoglobin A1c(HbA1c)89,95%CI(-1.35,-0.44),P=0.0001〗,2-hour postprandial blood glucose value(2 h PG)[SMD=-0.90,95%CI(-1.27,-0.53),P<0.00001],Diabetes Self-efficacy Scale(DSES)score,[SMD=3.19,95%CI(2.52,3.86),P<0.00001].Conclusion Available evidence suggests that visiting interventions can improve glycemic markers and self-efficacy levels in elderly patients with diabetes.
