血栓弹力图在妊娠晚期孕妇凝血功能状态评估中的应用

摘　　要：目的分析血栓弹力图在妊娠晚期孕妇凝血功能状态评估中的应用价值。方法选取70例妊娠晚期孕妇,根据妊娠情况分为高危妊娠孕妇组与正常孕妇组,各35例。均接受血栓弹力图和实验室凝血指标检查,比较两组凝血功能。结果两组的活化部分凝血活酶时间、凝血酶原时间、凝血酶时间、纤维蛋白原及D-二聚体水平比较,差异有统计学意义(P<0.05)。正常孕妇组凝血反应时间(R)、凝血形成时间(K)均高于高危妊娠孕妇组,凝血形成速率(Angle)、血凝块最大强度(MA)、纤维蛋白溶解率(LY30)和综合凝血指数(CI)参数均低于高危妊娠孕妇组,差异有统计学意义(P<0.05)。凝血酶原时间、活化部分凝血酶原时间、纤维蛋白原和D-二聚体与血栓弹力图参数具有相关性(P<0.05)。结论血栓弹力图可以反映妊娠晚期孕妇的凝血功能状态,有利于预防血栓发生。Objective To analyze the application value of thromboelastography in the evaluation of coagulation function status of pregnant women in the third trimester.Methods 70 pregnant women in the third trimester were selected and divided into high-risk pregnant women group and normal pregnant women group,35 cases each.Thromboelastogram and laboratory coagulation index were examined to compare the coagulation function of the two groups.Results There were significant differences in activated partial thromboplastin time,prothrombin time,thrombin time,fibrinogen and D-dimer levels between the two groups(P<0.05).The coagulation reaction time(R)and coagulation formation time(K)in the normal pregnant women group were higher than those in the high-risk pregnant women group,and the parameters of coagulation rate(Angle),maximum clot intensity(MA),fibrinolysis rate(LY30)and comprehensive coagulation index(CI)were lower than those in the high-risk pregnant women group,with statistical significance(P<0.05).Prothrombin time,activated partial prothrombin time,fibrinogen and D-dimer were correlated with thromboelastogram parameters(P<o.05).Conclusion Thromboelastography can reflect the state of coagulation function of pregnant women in the third trimester,which is beneficial to effectively prevent thrombosis.
