宫腔用交联透明质酸钠凝胶对预防宫腔操作术后宫腔粘连的效果观察

摘　　要：目的探讨宫腔用交联透明质酸钠凝胶对预防宫腔操作术后宫腔粘连的效果。方法选取78例行宫腔操作术的患者,按照随机数字抽样法分为对照组和观察组,每组39例。对照组行宫腔操作术后应用常规抗生素及促宫缩治疗,观察组行宫腔操作术后使用宫腔用交联透明质酸钠凝胶,同时实施常规抗生素、促宫缩治疗。比较两组术后康复相关指标及术后宫腔粘连发生情况。结果观察组术后腹痛持续时间、阴道流血时间均显著短于对照组,术后阴道流血量显著少于对照组,差异均有统计学意义(P<0.05);随诊显示,观察组术后月经复潮时间显著原于对照组,术后3个月子宫内膜厚度显著厚于对照组,差异均有统计学意义(P<0.05)。观察组术后3个月宫腔粘连IUA评分(1.92±0.78)分显著低于对照组的(3.65±1.42)分,观察组术后宫腔粘连发生率5.13%(2/39)显著低于对照组的20.51%(8/39),差异有统计学意义(P<0.05)。结论宫腔用交联透明质酸钠凝胶可有效降低宫腔操作术后宫腔粘连发生风险,并可加速患者康复进程,临床应用效果可靠。Objective To investigate the effect of Cross-linked sodium hyaluronate gel on preventing intrauterine adhesion after intrauterine operation,Methods 78 patients undergoing intrauterine operation were divided into control group and observation group according to random number sampling method,with 39 cases in each group.The control group was treated with conventional antibiotics and uterine stimulating therapy after uterine operation,and the observation group was treated with uterine Cross-linked sodium hyaluronate gel after uterine operation,while conventional antibiotics and uterine stimulating therapy were administered.The indexes of postoperative rehabilitation and the incidence of intrauterine adhesions were compared between the two groups.Results The duration of postoperative abdominal pain and vaginal bleeding in the observation group were significantly shorter than those in the control group,and the amount of postoperative vaginal bleeding was significantly lower than that in the control group,with statistical significance(P<0.05).Follow-up showed that the period of menstrual rehydration in the observation group was significantly shorter than that in the control group,and the endometrial thickness was thicker 3 months after surgery,with statistical significance(P<0.05).The IUA score of intrauterine adhesions in the observation group(1.92±0.78)points was significantly lower than that in the control group(3.65±1.42)points three months after surgery,and the incidence of intrauterine adhesions in the observation group 5.13%(2/39)was significantly lower than that in the control group 20.51%(8/39),with statistical significance(P<0.05).Conclusion Cross-linked sodium hyaluronate gel can effectively reduce the risk of intrauterine adhesions after intrauterine operation,and accelerate the recovery process of patients,and the clinical application effect is reliable.
