鼻内镜下全组鼻窦开放术治疗嗜酸性粒细胞性慢性鼻窦炎伴鼻息肉的疗效分析

摘　　要：目的探讨不同鼻内镜手术治疗嗜酸性粒细胞性慢性鼻窦炎伴鼻息肉的临床效果。方法选取2019年9月至2022年3月因嗜酸性粒细胞性慢性鼻窦炎伴鼻息肉在佳木斯大学附属第一医院耳鼻咽喉科行鼻内镜手术的患者96例,采用随机数字表法分为根治组(行鼻内镜下全组鼻窦开放术治疗)和对照组(行功能性鼻内镜鼻窦开放术治疗),每组48例。比较两组患者的手术疗效、鼻气道阻力值、鼻内镜评分、嗅觉功能、术后感染及复发情况。结果两组的手术疗效差异无显著性(P>0.05)。术前两组气道阻力值、鼻内镜评分、嗅觉功能比较差异无显著性(P>0.05);术后两组气道阻力值、鼻内镜评分、嗅觉功能均明显优于术前,且根治组明显优于对照组,差异有显著性(P<0.05)。两组术后感染及复发情况比较差异均无显著性(P>0.05)。结论鼻内镜下全组鼻窦开放术治疗嗜酸性粒细胞性慢性鼻窦炎伴鼻息肉可有效降低鼻腔阻力,改善嗅觉功能,且不增加术后感染及复发的风险。Objective To investigate the clinical effects of different endoscopic operations in the treatment of eosinophilic chronic sinusitis with nasal polyps.Method From September 2019 to March 2022,a total of 96 patients undergoing endoscopic sinus surgery in the Otolaryngology Department of the First Affiliated Hospital of Jiamusi University for chronic sinusitis with nasal polyps were selected and divided into radical treatment group(radical sinus operation)and control group(functional nasal endoscopic surgery)by random number table method,with 48 cases in each group.The surgical effect,nasal airway resistance,nasal endoscopic score,olfactory function,postoperative infection and recurrence were compared between the two groups.Result The curative effect of radical treatment group was not significantly higher than that of control group(P>0.05).There were no significant differences in airway resistance,nasal endoscopic score and olfactory function between the two groups before surgery(P>0.05).Airway resistance,nasal endoscopy score and olfactory function of both groups were significantly better than those before surgery,and the radical treatment group was significantly better than the control group,with significant differences(P<0.05).There were no significant differences in postoperative infection and recurrence between the two groups(P>0.05).Conclusion Endoscopic sinus operation for the treatment of eosinophilic chronic sinusitis with nasal polyps can effectively reduce nasal resistance,improve olfactory function,and do not increase the risk of postoperative infection and recurrence.
