摘 要:目的探讨全体素测量磁共振扩散加权成像单指数模型和体素内不相干运动(IVIM)模型参数在肝癌诊断中的应用价值。方法选取医院2021年1月至2022年12月收治的肝细胞癌患者120例,均行核磁共振成像(MRI)平扫、IVIM-扩散加权成像(DWI),获得单指数模型、双指数模型及拉伸指数模型参数,获取最大病灶感兴趣区(ROI),避开肝内胆管组织,计算各参数的平均值。评价各项技术所呈现的图像及参数;比较不同扫描技术获得的参数在肝癌病理类型分级中的差异。结果肝癌E-S分级为E-S1、E-S2、E-S3、E-S4患者的表观扩散系数(ADC)和慢速表观扩散系数(ADC slow)均逐渐降低,灌注分数(f)逐渐升高(P<0.05);E-S1患者的快速表观扩散系数(ADC fast)低于E-S2患者(P<0.05);E-S2、E-S3、E-S4患者的ADC fast逐渐降低(P<0.05)。肝癌不同E-S分级诊断中ADC和ADC slow的灵敏度均显著高于ADC fast和f,ADC fast的灵敏度显著高于f(P<0.05);ADC slow和f的特异度均显著高于ADC和ADC fast(P<0.05)。结论全体素测量磁共振扩散加权成像单指数模型和IVIM模型参数在肝癌诊断中的应用价值高。Objective To investigate the application value of monoexponential model and intravoxel incoherent motion(IVIM)model parameters in the diagnosis of liver cancer using whole-element measurement magnetic resonance diffusion weighted imaging.Methods A total of 120 patients with liver cancer admitted to the hospital from January 2021 to December 2022 were selected,all of whom underwent magnetic resonance imaging(MRI)plain scan and IVIM-diffusion weighted imaging(DWI).The parameters of the single-index model,double-index model and stretching index model were obtained,and the region of interest(ROI)of the largest lesion was obtained,the intrahepatic bile duct tissue was avoided.The average value of each parameter was calculated.The images and parameters presented by various technologies were evaluated.The differences in parameters obtained by different scanning techniques in the pathological classification of liver cancer were compared.Results The apparent diffusion coefficient(ADC)and slow apparent diffusion coefficient(ADC slow)of patients with liver cancer E-S grading of E-S1,E-S2,E-S3,E-S4 gradually decreased,while the perfusion fraction(f)gradually increased(P<0.05).The fast apparent diffusion coefficient(ADC fast)of E-S1 patients was lower than that of E-S2 patients(P<0.05).The ADC fast of patients with E-S2,E-S3,and E-S4 gradually decreased(P<0.05).The sensitivity of ADC and ADC slow in the diagnosis of liver cancer with different E-S grades was significantly higher than that of ADC fast and f.The sensitivity of ADC fast was significantly higher than that of f(P<0.05).The specificity of ADC slow and f was significantly higher than that of ADC and ADC fast(P<0.05).Conclusion The monoexponential model and IVIM model parameters of diffusion weighted imaging with whole-element measurement have high application value in the diagnosis of liver cancer.
