血清早期生长应答因子1与2型糖尿病视网膜病变的相关性

摘  要:目的探讨血清早期生长应答因子1(Egr-1)水平与2型糖尿病(T2DM)视网膜病变(DR)的相关性。方法选取医院2020年1月至2022年6月收治的T2DM患者84例,按是否合并DR分为A组(合并DR,40例)和B组(不合并DR,44例),另选取同期健康体检者48例作为对照组(C组)。常规检测3组患者的血糖、肾功能等指标水平,比较血清Egr-1水平。结果B组、C组超敏C反应蛋白(hs-CRP)、空腹血糖(FBG)、糖化血红蛋白(HbA1C)水平明显高于A组,且C组明显高于B组(P<0.05);C组尿素氮(BUN)、Egr-1明显高于A组和B组(P<0.05),且B组与A组相当(P>0.05);C组24 h尿微量白蛋白水平明显高于B组(P<0.05),胰岛素抵抗指数(HOMA-IR)与B组相当(P>0.05)。Egr-1与24 h尿微量白蛋白,BUN,FBG,HbA1C,hs-CRP水平及DR分期均呈正相关性(r=0.639,0.567,0.688,0.601,0.939,0.712;P<0.05)。随着DR分期的增加,血清Egr-1水平逐渐增加,而Ⅰ、Ⅱ期DR患者Egr-1水平相当(P>0.05)。结论血清Egr-1可能作为DR的一种新型炎性生物学标志物用于临床诊断。Objective To investigate the correlation between the level of serum early growth response protein 1(Egr-1)and type 2 diabetes mellitus(T2DM)retinopathy(DR).Methods A total of 84 T2DM patients admitted to the hospital from January 2020 to June 2022 were selected and divided into the group A(40 patients with DR)and the group B(44 patients without DR)according to whether DR occurred.Additionally,48 healthy individuals who underwent physical examination during the same period were selected as the control group(group C).The blood glucose,renal function and other indicators in three groups were tested routinely,and the serum Egr-1 level in three groups was compared.Results The levels of high-sensitivity C-reactive protein(hs-CRP),fasting blood glucose(FBG)and glycosylated hemoglobin(HbA1C)in groups B and C were significantly higher than those in group A,and those in group C were significantly higher than those in group B(P<0.05).Blood urea nitrogen(BUN),Egr-1 in group C was significantly higher than that in groups A and B(P<0.05),and that in group B was similar to that in group A(P>0.05).The 24-hour urinary microalbumin level in group C was significantly higher than that in group B(P<0.05),the homeostatic model assessment of insulin resistance(HOMA-IR)in group C was similar to that in group B(P>0.05).Egr-1 was positively correlated with 24-hour urinary microalbumin,BUN,FBG,HbA1C,hs-CRP levels and DR staging(r=0.639,0.567,0.688,0.601,0.939,0.712;P<0.05).As the DR stage increased,the serum Egr-1 level gradually increased,and the Egr-1 level in patients with stageⅠandⅡof DR were similar.Conclusion Serum Egr-1 may serve as a novel inflammatory biomarker for clinical diagnosis of DR.
