门诊血透室一体化药品管理流程优化效果评价

摘　　要：目的提高门诊维持性血液透析患者的药品管理规范率及使用安全性。方法医院自2020年1月起采用医院信息系统(HIS)对门诊维持性血液透析患者进行住院化管理,联合多部门实施血透室一体化药品管理。随机选取管理前(2019年1月至12月)在门诊行维持性血液透析的35例患者为对照组,管理后(2020年1月至12月)35例患者为观察组,比较两组患者的药品种类出错率、遗忘返院带药率、返院带药失效率、患者满意度及护理质量评分。结果观察组药品种类出错、遗忘返院带药和返院带药失效发生率均显著低于对照组,护理质量评分及满意度显著高于对照组(P<0.05)。结论一体化药品管理流程应用于门诊血透室效果较理想,能显著改善门诊血透室的护理服务质量,降低药品差错事件的发生率;保障安全、节约资源;改善患者就医感受,提升患者满意度。Objective To improve the standardized drug management rate and safety of outpatient maintenance hemodialysis patients.Methods The outpatient maintenance hemodialysis patients were given hospitalization management in the hospital by hospital information system(HIS)since January 2020,and integrated drug management in hemodialysis rooms was implemented by collaborating with multiple departments.A total of 35 patients underwent maintenance hemodialysis in the outpatient department before management(January to December 2019)were selected as the control group,and 35 patients who underwent management(January to December 2020)were selected as the observation group.The drug type error rate,forgetting bring back drugs rate,return to hospital medication failure rate,patient satisfaction,and nursing quality scores in the two groups were compared.Results The incidence of drug type errors,forgetting to bring back drugs,and return to hospital medication failure rate in the observation group were significantly lower than those in the control group,and the nursing quality scores and satisfaction were significantly higher than those in the control group(P<0.05).Conclusion The application of integrated drug management process in outpatient hemodialysis rooms has an ideal effect,which can significantly improve the quality of nursing services in outpatient hemodialysis rooms and reduce the incidence of drug error events,ensure safety and save resources,improve patients'medical experience and enhance patient satisfaction.
