摘　　要：目的评价外周血中性粒细胞与淋巴细胞比值(NLR)、中性粒细胞与补体C3、C4比值(NC3R、NC4R)以及C反应蛋白与白蛋白比值(CRP/Alb)对系统性红斑狼疮(SLE)患者疾病活动度的临床价值。方法选取江苏大学附属武进医院2019年1月至2023年6月收治的54例SLE患者,根据SLE疾病活动评分(SLEDAI)分为疾病活动组(24例)和缓解组(30例),比较两组NLR、NC3R、NC4R以及CRP/Alb比值差异,分析NLR、NC3R、NC4R以及CRP/Alb比值与SLEDAI指数的相关性,绘制受试者工作特征曲线(ROC)评价NLR、NC3R、NC4R对SLE疾病活动度的预测价值。结果疾病活动度组NLR、NC3R和NC4R均明显高于缓解组(P<0.01),而CRP/Alb两组间差异无统计学意义(P>0.05);相关性分析结果显示,NLR、NC3R和NC4R与患者SLEDAI评分呈正相关(P<0.01),CRP/Alb与SLEDAI评分无相关性(P>0.05);ROC曲线显示,NLR预测SLE活动度的曲线下面积(AUC)为0.760,取最佳截断值时,灵敏度为75.0%、特异度为70.0%;NC3R预测SLE活动度的AUC为0.815,灵敏度为79.2%、特异度为83.3%;NC4R预测SLE活动度的AUC为0.715,灵敏度为54.2%、特异度为83.3%。结论NLR、NC3R和NC4R可能为SLE疾病活动度病情评估提供参考依据。Objective To evaluate the clinical value of neutrophil to lymphocyte ratio(NLR),neutrophil to complement C3,C4 ratio(NC3R,NC4R)and C-reactive protein to albumin ratio(CRP/Alb)in assessing the disease activity of systemic lupus erythematosus(SLE).Methods Fiftyfour SLE patients treated in Wujin Hospital Affiliated with Jiangsu University from January 2019 to June 2023 were selected,were divided into the active group(24 cases)and the remission group(30 cases)according to SLE disease score activity index(SLEDAI).The differences between the two groups of NLR,NC3R,NC4R and CRP/Alb ratio were compared and the correlation with SLEDAI index was analyzed,Receiver operating characteristic curve(ROC)was performed to evaluate the predictive value of NLR,NC3R and NC4R for SLE disease activity.Results The NLR,NC3R and NC4R in the active group were significantly higher than those of the remission group(P<0.01),while there was no significantly difference in CRP/Alb between the two groups(P>0.05).Correlation analysis showed that NLR,NC3R and NC4R were significantly positively correlated with SLEDAI score(P<0.01),while CRP/Alb was not correlated with SLEDAI score(P>0.05).The ROC curve showed that the area under the curve(AUC)of NLR in predicting the disease activity of SLE was 0.760,and the sensitivity was 75.0%,specificity was 70.0%when the optimal cut-off value was obtained.The AUC of NC3R in predicting the disease activity of SLE was 0.815,and the sensitivity was 79.2%,specificity was 83.3%.The AUC of NC4R in predicting the disease activity of SLE was 0.715,and the sensitivity was 54.2%,specificity was 83.3%.Conclusions The NLR,NC3R and NC4R can be used to evaluate the disease activity of SLE patients.
