舒芬太尼配伍罗哌卡因椎管内麻醉对无痛分娩产妇疼痛评分及总产程时间的影响

摘　　要：目的评价在产妇实施无痛分娩时应用舒芬太尼配伍罗哌卡因椎管内麻醉的效果。方法纳入2021年5月至2022年6月在我院实施无痛分娩的产妇68例,根据麻醉方式不同分为对照组和试验组,两组各34例,对照组采用罗哌卡因椎管内麻醉,试验组采用舒芬太尼配伍罗哌卡因椎管内麻醉,对比两组临床效果。结果试验组镇痛起效、完全阻滞时间短于对照组,镇痛持续时间长于对照组(P<0.05)。试验组镇痛后1 h与胎儿娩出时疼痛评分低于对照组(P<0.05)。试验组各产程时间均短于对照组(P<0.05)。试验组自然分娩率更高(P<0.05)。结论在产妇实施无痛分娩时,应用舒芬太尼配伍罗哌卡因椎管内麻醉,阵痛效果较好,可减轻疼痛程度,缩短产程时间,提高自然分娩率。Objective To evaluate the effect of intravertebral anesthesia of ropivacaine with sufentanil in labor and delivery.Methods A total of 64 cases of women who underwent painless labor in our hospital from May 2021 to June 2022 were included,divided into a control group and an experimental group according to the different anesthesia modalities,34 cases in each group,with ropivacaine intrathecal anesthesia used in the control group,and sufentanil with ropivacaine intrathecal anesthesia used in the experimental group,and the clinical effects of the two groups were compared.Results The onset of analgesia and complete block time of the experimental group were shorter than that of the control group,and the duration of analgesia was longer than that of the control group(P<0.05).The pain scores of the experimental group were lower than those of the control group at 1 h after analgesia and at the time of fetal delivery(P<0.05).The duration of all stages of labor was shorter in the experimental group than in the control group(P<0.05).The rate of spontaneous delivery was higher in the experimental group(P<0.05).Conclusions In the implementation of painless labor,the application of sufentanil with ropivacaine intrathecal anesthesia in labor has a better paroxysmal effect,which can reduce the pain level,shorten the duration of labor,and increase the rate of natural delivery.
