调肺运脾推拿法治疗痰湿蕴肺型小儿慢性咳嗽护理中对患儿中医证候评分、肺通气功能的影响

摘　　要：目的探讨调肺运脾推拿法治疗痰湿蕴肺型小儿慢性咳嗽护理中对患儿中医证候评分、肺通气功能的影响。方法选择2021年6月至2023年6月厦门市思明区莲前街道社区卫生服务中心收治的120例痰湿蕴肺型慢性咳嗽病例,按照随机数字表法的分组要求,120病例分为两组,包括观察组、对照组均有60例,对照组患儿以盐酸氨溴索为主要的治疗方案,观察组患儿结合调肺运脾推拿法治疗,比较两组患儿的临床治疗效果,咳嗽程度、时间以及胸闷、纳呆中医证候评分,治疗前后的肺通气功能情况。结果经过治疗观察组有效率达到96.67%,高于对照组的76.67%(P<0.05)。相比治疗前,两组患儿中医证候积分及R5、R20、R5-R20各项肺通气功能指标均得到改善,各指标(评分)降低(P<0.05),观察组患儿中医症候评分与肺通气功能指标均低于对照组(P<0.05)。结论为痰湿蕴肺型小儿慢性咳嗽病例实施调肺运脾推拿实施治疗可显著降低患儿的中医症候评分,改善患儿肺通气功能,提高临床治疗效果。Objective To explore the effects of regulating lung and spleen massage on TCM syndrome score and lung ventilation function in treating children with phlegm-dampness and lung chronic cough.Methods A total of 120 children with phlegm-damp-pulmonary chronic cough admitted to our hospital from June 2021 to June 2023 were selected and divided into observation group and control group by random number table method,60 patients in each group.Ambroxol hydrochloride was used as the main treatment regimen for children in the control group,and patients in the observation group were treated with lung regulating and spleen promoting massage.Clinical treatment effects of the two groups were compared.Cough degree,duration,chest tightness,TCM syndrome score,pulmonary ventilation function of children before and after treatment.Results After treatment,the effective rate was 96.67%,which was higher than that of the control group(P<0.05).Before comparative treatment,TCM syndrome score and pulmonary ventilation function indexes of R5,R20 and R5-R20 were improved in both groups,and all indexes(scores)were decreased(P<0.05).The improvement was more obvious in observation group than in control group,and TCM syndrome score and pulmonary ventilation function indexes were lower than those in control group(P<0.05).Conclusions The treatment of chronic cough with phlegm-dampness in children by regulating lung and spleen massage can significantly reduce the score of TCM symptoms,improve the pulmonary ventilation function and improve the clinical therapeutic effect,which has high application value.
