摘 要:目的:比较磁共振检查序列中的动态增强检查和弥散加权成像(DWI)在早期小肝癌及其他肝脏结节性病变的检出率及诊断符合率的大小,以获取肝脏结节病变最适宜扫描方案。方法:选取2016年1月—2021年12月于山西省临汾市人民医院就诊患者115例,肝结节病灶数共150个,统计动态增强磁共振成像(DCE-MRI),DWI及两者联合使用对病变的检出率及诊断符合率。结果:选取150个病灶,其中DCE-MRI扫描序列检出发现病灶127个,检出率84.7%,DWI共发现病灶120个,检出率80.0%,DCE-MRI和DWI联合使用共发现病灶135个,检出率90.0%,效果显著优于两种方法独立使用(χ^(2)=6.138,P=0.046)。115例患者中,DCE-MRI扫描诊断符合99个,正确率86%,DWI诊断符合83个,正确率72%,前者对病灶的定性正确率显著优于后者(P=0.009)。DCE-MRI和DWI联合使用诊断符合110个,正确率达95%,效果显著优于两种方法独立使用(χ^(2)=24.656,P<0.05)。结论:两种技术联合使用可以显著提高肝癌及肝结节性病变检出率及定性率,在临床工作中值得被广泛推广。Objective To obtain the optimal scanning protocol for liver lesions by comparing the detection rate and diagnostic compliance rate of dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)and diffusion-weighted imaging(DWI)in liver cancer and other liver nodular lesions.Methods From January 2016 to December 2021,115 patients were treated in Linfen People's Hospital of Shanxi Province,with a total of 150 liver sarcoidal lesions,statistical DCE-MRI,DWI and the combination of the two on the detection rate and diagnostic compliance rate of lesions.Results Among the 150 lesions,a total of 127 lesions with a detection rate of 84.7%were found by the combination of DCE-MRI,120 lesions were found by DWI,with a detection rate of 80%,and 135 lesions were found with a detection rate of 90%by DCE-MRI and DWI,which was significantly better than that of the two methods(χ^(2)=6.138,P=0.046)alone.Among the 115 patients,99 diagnosis of DCE-MRI were confirmed with a correct rate of 86%,and 83 diagnosis of DWI were diagnosed with a correct rate of 72%,and the qualitative accuracy of the former for lesions was significantly better than that of the latter(P=0.009).The diagnosis of the combination of DCE-MRI and DWI combined use was 110 with a 95%accuracy rate,which was significantly better than the independent use of the two methods(χ^(2)=24.656,P<0.05).Conclusions The combined use of the two technologies can significantly improve the detection rate and qualitative rate of liver cancer and liver nodular lesions,and it is worth being widely promoted in clinical work.
