补充微量元素对胃切除术后禁食患者营养状况的影响

摘　　要：目的:探讨肠外营养中微量元素对胃切除术后禁食患者营养状况的影响。方法:回顾性选取2014年1月至2017年4月该院收治的胃切除术后采用全肠外营养的患者60例,根据肠外营养中是否添加微量元素分为观察组(30例)和对照组(30例)。观察组患者使用含微量元素的肠外营养,对照组患者使用不含微量元素的肠外营养。术后每隔3 d进行体格检查,并检测营养状态指标。结果:观察组患者术后第4日的血钠水平(P<0.05)、血钙水平(P<0.05)显著高于对照组,血镁水平明显低于对照组(P<0.001);术后第7日的心率、血糖水平显著低于对照组(P<0.05);而术后第4日的白蛋白水平(P<0.01)、前白蛋白水平(P<0.05)明显高于对照组,上述差异均有统计学意义。观察组患者白蛋白、前白蛋白水平在第7日被对照组追平,组间差异无统计学意义(P>0.05)。结论:微量元素对胃切除术后禁食患者电解质、心率和血糖等均有显著影响,肠外营养中添加微量元素有利于患者身体与生理机能的康复和稳定。OBJECTIVE:To probe into the effects of trace elements supplementation in parenteral nutrition on the nutritional status of fasting patients after gastrectomy.METHODS:A total of 60 patients received total parenteral nutrition after gastrectomy admitted into this hospital from Jan.2014 to Apr.2017 were retrospectively selected to be divided into observation group(30 cases)and control group(30 cases)according to whether or not the trace elements were added into parenteral nutrition.The observation group was given parenteral nutrition with trace elements,the control group was given parenteral nutrition without trace elements.Postoperative physical examinations were performed and indicators of nutritional status were tested every other 3 days.RESULTS:The blood sodium(P<0.05)and blood calcium(P<0.05)levels were significantly higher and the blood magnesium(P<0.001)level was significantly lower in the observation group than those in the control group on the 4th day after surgery;the heart rate and blood glucose of the observation group were significantly lower than those of the control group on the 7th day after surgery(P<0.05);the albumin(P<0.01)and prealbumin(P<0.05)of the observation group were significantly higher than those of the control group on the 4th day after surgery,the above differences were all statistically significant.The albumin and prealbumin levels of the observation group were equalized by the control group on the 7th day,the differences of between-group comparison were not statistically significant(P>0.05).CONCLUSIONS:Trace elements have significant effects on the electrolyte,heart rate and blood glucose in fasting patients after gastrectomy,addition of trace elements in parenteral nutrition is beneficial to the recovery and stabilization of patients’physical and physiological function.
